摘　　要：目的探讨全髋关节置换术治疗老年陈旧性股骨颈骨折的疗效。方法 2013年9月—2017年9月对34例伴有下肢短缩老年陈旧性股骨颈骨折患者实施生物型全髋关节置换,记录术后1、3、6、12个月及以后每年X线复查假体位置及骨长入情况,采用Harris评分评估术后患者髋功能恢复情况。结果术后随访6～48个月,无并发坐骨神经损伤、术后髋关节脱位,无髋关节假体松动及假体周围骨折等并发症。术后Harris评分明显高于术前,术后患者肢体缩短明显短于术前,差异有统计学意义(P <0.05)。结论生物型假体髋关节置换术能有效治疗老年陈旧性股骨颈骨折,减少病残率和术后并发症的发生率,明显改善髋关节功能,提高生活质量。Objective To investigate the effect of total hip arthroplasty in the treatment of old femoral neck fractures in the elderly.Methods From September 2013 to September 2017,34 cases of old femoral neck fracture with short contraction of the lower extremities were treated with total hip arthroplasty.The position of the prosthesis and the length of bone were recorded at 1,3,6,12 months after the operation and X-ray examination every year,and the recovery of the hip function after the operation was evaluated by Harris score.Results The average follow-up was 6~48 months after operation.There were no complications such as sciatic nerve injury,postoperative dislocation of hip joint,no hip prosthesis loosening and periprosthetic fracture.Preoperative Harris score was significantly less than postoperative score,and postoperative limb shortening was significantly shorter than that before operation(P<0.05).Conclusion Bioprosthesiship replacement can effectively treat old femoral neck fracture,reduce the incidence of disability and postoperative complications,improve the function of hip joint and improve the quality of life.
