剖宫产术不缝合皮下脂肪组织的效果研究

摘　　要：目的探讨腹部横切口剖宫产术不缝合皮下脂肪组织的临床应用效果。方法选行剖宫产术分娩的产妇1 000例,分为观察组(例数=500)和对照组(例数=500)。两组均常规行剖宫产术,对照组手术完毕后常规缝合皮下脂肪组织,观察组不缝合皮下脂肪组织。比较两组产妇产后的切口愈合情况,手术时间、术中出血量、术后排气时间、术后疼痛程度和术后住院时间等手术指标及术后切口不良愈合情况。结果观察组腹部切口甲级愈合率为92.4%,高于对照组的76.2%,P <0.05。观察组手术时间明显短于对照组,术后疼痛程度明显轻于对照组,术后住院时间明显短于对照组,P <0.05。观察组产妇术后切口感染1例;对照组产妇术后皮下血肿4例、切口感染6例、脂肪液化4例。观察组术后皮下血肿,切口感染,脂肪液化发生率为0.2%,低于对照组的2.8%,P <0.05。结论腹部横切口剖宫产术不缝合皮下脂肪组织可促进产妇切口愈合。Objective To investigate the clinical effect of abdominal transverse incision cesarean section without suture subcutaneous adipose tissue.Methods 1 000 parturients were divided into observation group(cases=500)and control group(cases=500).Both groups were routinely performed cesarean section,the control group was routinely sutured subcutaneous adipose tissue after surgery,the observation group was not sutured subcutaneous adipose tissue.The postoperative wound healing,operative time,intraoperative bleeding volume,postoperative exhaust time,postoperative pain and postoperative hospital stay were compared between the two groups.Results The I healing rate of the abdominal incision in the observation group was 92.4%,significantly higher than that in the control group(76.2%),and P<0.05.The operation time of the observation group was shorter than that of the control group,and the postoperative pain degree was lighter than that of the control group.The postoperative hospitalization time was shorter than that of the control group,P<0.05.There were 1 case of incision infection in observation group,4 cases of subcutaneous hematoma,6 cases of incision infection and 4 cases of fat liquefaction in control group.The incidence of subcutaneous hematoma,incision infection and fat liquefaction in the observation group was 0.2%,significantly lower than that in the control group(2.8%),P<0.05.Conclusion Abdominal transverse incision cesarean section without suture subcutaneous adipose tissue can promote the healing of maternal incision.
