摘　　要：目的总结腮腺腺淋巴瘤(Warthin瘤)的临床特点及声像图特征,以提高超声诊断的准确性。方法回顾2012年1月1日—2018年8月15日在我院经病理证实腮腺Warthin瘤患者71例(共94个病灶)的临床资料、超声图像,并与病理结果对照分析,总结其临床特点、声像特征,分析超声诊断情况。结果 71例腮腺Warthin瘤,男女比例为13.2∶1,平均年龄(59.24±9.61)岁;吸烟史54.9%(39/71);单侧腮腺单发病灶为主74.6%(53/71);病灶位于浅叶后下极66.0%(62/94)。超声分型:实性肿块型、囊实性肿块型、囊性肿块型,分别占70.2%(66/94)、24.5%(23/94),5.3%(5/94)。术前超声与病理诊断良性肿瘤符合率97.2%(69/71),WT诊断准确率25.4%(18/71)。结论腮腺Warthin瘤多发生在年龄50～70岁老年男性,多有较长吸烟史,可有疼痛及消长史,多位于腮腺后下极,超声为椭圆形,界清,包膜完整,内呈网格样回声,常伴液化,血流信号丰富。通过Warthin瘤的超声表现与病理结果对照,分析WT声像特征,对提高术前超声诊断WT准确率有重要的价值。Objective To summarize the clinical features and sonographic features of Warthin tumor of the Parotid glands,in oder to improve the accuracy of ultrasound diagnosis.Methods From January 1,2012 to August 15,2018,the clinical data and ultrasound images of 71 patients(with 94 lesions)of parotid Warthin tumor confirmed by surgery and pathology in our hospital were retrospectively analyzed,inclouding clinical characteristics,ultrasounic features and classification.The clinical and sonographic features were summarized and compared with the pathological results,and the diagnosis of ultrasound was analyzed.Results 71 cases of parotid Warthin tumor,M/F ratio 13.2:1,mean age(59.24±9.61),smoking history 54.9%(39/71),singlesided single lesion74.6%(53/71),lower posterior poles 66.0%(62/94).Ultrasound classification:solid mass type,cystic and mass type,cystic type,accounting for 70.2%(66/94),24.5%(23/94),5.3%(5/94).The coincidence rate of preoperative ultrasound and pathological diagnosis of benign tumors was 97.2%(69/71),and the diagnostic accuracy of WT was 25.4%(18/71).Conclusion Warthin tumors of the parotid gland occur mostly in men over 50 years old,may have a history of smoking,pain and growth-diminution and mostly in the lower posterior pole.The ultrasound appears to be round or elliptical,intact membrane uneven internal echo of grid-like,often accompanied with liquefaction,and rich blood flow.
