摘 要:目的研究数字化设计和3D打印技术辅助胫骨高位导向截骨的方法,探讨其可行性、优越性、准确性。方法采集30例临床膝关节骨性关节炎患者的下肢连续薄层CT扫描的Dicom格式图像,Mimics软件读取原始数据分别予以三维模型重建、数字化设计。确定胫骨高位截骨位置,设计带钉道的截骨导向模块。3D打印骨骼、导向模块,模拟胫骨高位截骨手术,观察导向模块卡位、导针位置情况,评价效果。结果 30例骨骼模型与胫骨高位截骨导向模块卡位简便、贴合紧密,能良好嵌合,在应用时操作稳定,可以很好指引放置导针、导向截骨。结论数字化设计的3D打印导向模块能良好辅助胫骨高位截骨,该方法可行,安全性、准确率高。Objective To study digital design and three dimensional printing(3D printing)technology assisting high tibial guided osteotomy and explore its feasibility,superiority and accuracy.Methods The Dicom format images of lower limb continuous thin layer CT scanning of patients with knee osteoarthritis from 30 clinical cases were collected,and the raw data were imported to the Mimics software for 3D module reconstruction and digital design.After determining positions of high tibial osteotomy,the osteotomy guide-modules with pin tracts were designed.Relative bones and guide-modules were then 3D printed for the simulation surgery of high tibial osteotomy.By observing the clamping positions of guide-modules and the positions of guiding pins,the effects were evaluated.Results The bone models and the high tibial osteotomy guide-modules from all 30 clinical cases were easy to clamp in position and can be well-fitted tightly.They can guarantee the operation stability in the real-world application and work well for guiding pin placement and osteotomy.Conclusion The 3D printed guide-modules using digital design can be used to assist high tibial osteotomy.The method is feasible,safe and accurate.
