核素^(90)Sr-^(90)Y敷贴联合盐酸普萘洛尔治疗婴幼儿皮肤血管瘤临床分析

摘　　要：目的探讨核素^(90)Sr-^(90)Y敷贴联合盐酸普萘洛尔治疗婴幼儿皮肤血管瘤的临床疗效。方法研究对象选自2010年2月至2016年8月我院收治的92例婴幼儿皮肤血管瘤患儿,根据患儿的入院顺序做编号,采取随机数字表法分成两组,每组46例。两组均采取核素^(90)Sr-^(90)Y敷贴治疗,观察组同时给予盐酸普萘洛尔治疗,比较两组临床疗效及不良反应发生情况,并于治疗前后检测两组血浆血管内皮细胞生长因子(VEGF)、碱性成纤维细胞生长因子(bFGF)水平。结果观察组治疗后总有效率100%(46/46),与对照组84.78%(39/46)相比显著升高(均P<0.05)。治疗开始后8个月两组VEGF、bFGF水平均有显著下降(均P<0.05),但观察组降低更明显(P<0.05)。两组心率过缓、低血糖、肝肾损害、湿性放射性皮炎发生率比较均差异无统计学意义(均P> 0.05)。结论核素^(90)Sr-^(90)Y敷贴联合盐酸普萘洛尔治疗婴幼儿皮肤血管瘤可有效抑制血管瘤增殖,降低血浆VEGF、bFGF水平,疗效确切,但治疗过程中需做好不良反应的监测。Objective To explore clinical analysis of radionuclide 90 Sr-90 Y combined with propranolol hydrochloride in the treatment of infantile hemangioma.Methods 92 cases with infantile hemangioma were selected in our hospital from February 2010 to August 2016 as research objects.According to the order of admission,the children were divided into two groups randomly with 46 cases in each group.Radionuclide 90 Sr-90 Y was applied to both groups and propranolol hydrochloride was applied to observation group additionally.Two groups’clinical effect and adverse reaction were compared.The level of plasma endothelial cell growth factor(VEGF)and basic fibroblast growth factor(bFGF)before and after treatment were detected.Results The total effective rate was 100%in observation group(46/46),which was significantly higher than control group’s 84.78%(39/46)(P<0.01).Two groups’plasma VEGF,bFGF level decreased significantly from 8th month after treatment(P<0.05),and the level in observation group decreased more significantly(P<0.05).There was no significant difference between two groups in the incidence of heart rate,hypoglycemia,liver and kidney damage and wet radiation dermatitis(P>0.05).Conclusion Radionuclide 90 Sr-90 Y combined with propranolol hydrochloride could help patients with infantile hemangioma to inhibite hemangioma proliferation,and reduce the level of plasma VEGF,bFGF,which had a good effect.However,adverse reactions should be monitored in the treatment process.
