摘  要:目的观察玉香参附汤对心脏骤停大鼠心肌损伤的保护作用,并初步探讨其机制。方法制备心脏骤停/心肺复苏模型大鼠,随机分为对照组、模型组、依拉达奉组[5 mg/(kg·d)]、低[5 mg/(kg·d)]、中[10 mg/(kg·d)]、高[20 mg/(kg·d)]剂量玉香参附汤组,模型组及对照组灌胃等量0.9%氯化钠注射液。3 d后,统计各组大鼠存活情况,苏木素-伊红(HE)染色观察心肌组织结构;试剂盒检测心肌组织中活性氧(ROS)、丙二醛(MDA)、还原性谷胱甘肽(GSH)含量及过氧化氢酶(CAT)、超氧化物歧化酶(SOD)活性;蛋白印迹(WB)法检测心肌组织核转录因子(Nrf2)、NADH脱氢酶1(NQO1)、血红素氧合酶1(HO-1)蛋白表达。结果与对照组比较,模型组大鼠心肌组织出现心肌细胞坏死、炎性细胞浸润等病变,ROS水平、MDA含量显著升高,GSH含量及CAT、SOD活性显著降低,Nrf2蛋白核转移量明显减少,NQO1、HO-1蛋白表达量显著降低;与模型组比较,低、中、高剂量玉香参附汤组大鼠心肌组织病变明显减轻,ROS水平、MDA含量显著降低,GSH含量及CAT、SOD活性显著升高,Nrf2蛋白核转移量明显增多,NQO1、HO-1蛋白表达量显著增高(均P<0.05)。结论玉香参附汤可保护心脏骤停/心肺复苏所致大鼠心肌损伤,其机制可能是通过促进Nrf-2、NQO1、HO-1蛋白表达,减轻心肌组织氧化应激实现的。Objective:To investigate the protective effect of Yuxiang Shenfu Decoction on myocardial injury in rats with sudden cardiac arrest and to explore its mechanism.Methods:Sudden cardiac arrest/cardiopulmonary resuscitation model rats were prepared,and randomly divided into the control group,the model group,the Edaravone group[5mg/(kg·d)]and Yuxiang Shenfu Decoction groups-low[5mg/(kg·d)],middle[10mg/(kg·d)],high[20mg/(kg·d)].The model group and the control group were treated with the same amount of normal saline.After3d,the survival of rats in each group was statistically analyzed.HE staining was used to observe the myocardial tissue structure;ROS levels,MDA,reduced GSH and CAT and SOD activities in the myocardial tissues were detected by the kit;WB was used to detect the expressions of Nrf2,NQO1and HO-1in myocardial tissues.Results:Compared with the control group,myocardial tissues in the model group showed lesions such as cardiomyocyte necrosis and inflammatory cell infiltration;the level of ROS and the content of MDA increased significantly;the content of GSH and the activities of CAT and SOD decreased significantly;the nuclear transfer of Nrf2protein decreased significantly;the expressions of NQO1and HO-1proteins decreased significantly.Compared with the model group,the pathological changes of myocardial tissues in Yuxiang Shenfu Decoction groups were significantly lighten;the level of ROS and the content of MDA decreased significantly;the content of GSH and the activities of CAT and SOD increased significantly;the nuclear transfer of Nrf2protein increased significantly;the expressions of NQO1and HO-1proteins increased significantly(all P<0.05).Conclusion:Yuxiang Shenfu Decoction can protect the myocardial injury caused by sudden cardiac arrest/cardiopulmonary resuscitation in rats,whose mechanism may be achieved by reducing the oxidative stress of myocardial tissue by promoting the expressions of Nrf-2,NQO1and HO-1proteins.
