益气聪明汤对椎-基底动脉供血不足性眩晕患者血流速度与ET-1及CGRP水平影响

摘　　要：目的观察益气聪明汤治疗椎-基底动脉供血不足性眩晕患者的临床疗效并探讨其作用机制。方法患者92例随机分为两组,对照组予以盐酸氟桂利嗪胶囊口服治疗,观察组在对照组的基础上加用益气聪明汤加减治疗,连续服用2周。检测两组患者治疗前后的左侧椎动脉、右侧椎动脉及基底动脉的平均血流速度以及内皮素-1 (ET-1)、CGRP水平,评估两组患者的临床疗效以及中医证候积分。结果治疗后两组LV、ARVA、BA、CGRP水平显著高于治疗前(P <0.05),且观察组显著高于对照组(P <0.05);治疗后两组ET-1水平显著低于治疗前(P <0.05),且观察组显著低于对照组(P <0.05);治疗后3 d及7 d两组中医证候积分均低于治疗前(P<0.05),且观察组低于对照组(P <0.05);观察组总有效率93.50%优于对照组84.80%(P <0.05)。结论益气聪明汤能减轻椎-基底动脉供血不足性眩晕患者的临床症状,提高中医治疗疗效,并且能改善患者的血流速度,调节机体的ET-1和CGRP水平,安全可靠。Objective:To investigate the clinical efficacy of Yiqi Congming Decoction in the treatment of vertebrobasilar insufficiency vertigo and to explore its mechanism.Methods:92patients were randomly divided into two groups.The control group was treated with flunarizine hydrochloride capsule orally.The observation group was treated with Yiqi Congming Decoction on the basis of the control group for two weeks.The average blood flow velocities of left vertebral artery,right vertebral artery and basilar artery and the levels of ET-1and CGRP were measured before and after treatment,and the clinical efficacy and TCM syndrome scores of the two groups were evaluated.Results:After treatment,the levels of LV,ARVA,BA and CGRP in the two groups were significantly higher than those before treatment(P<0.05),and the level of ET-1in the observation group was significantly higher than that in the control group(P<0.05),while the level of ET-1in the two groups was significantly lower than that before treatment(P<0.05),and the scores of TCM symptoms in the observation group were significantly lower than those in the control group(P<0.05).Before treatment(P<0.05),and the observation group was lower than the control group(P<0.05);the total effective rate of the observation group was93.50%better than that of the control group84.80%(P<0.05).Conclusion:Yiqi Congming Decoction can alleviate the clinical symptoms of vertebrobasilar insufficiency vertigo patients,improve the curative effect of traditional Chinese medicine treatment,and improve the blood flow speed of patients,regulate the body′s ET-1and CGRP levels,which is safe and reliable.
