经尿道前列腺钬激光剜除术治疗良性前列腺增生临床效果观察

摘　　要：目的探讨良性前列腺增生采用经尿道前列腺钬激光剜除术治疗的临床效果。方法选择2016年3月至2017年9月本院收治的良性前列腺增生患者98例,随机分为对照组和观察组各49例。对照组行经尿道前列腺电切术,观察组行经尿道前列腺钬激光剜除术,随访6个月,比较两组手术相关指标、国际前列腺症状评分表(IPSS)、生活质量评分(QOL)、最大尿流率(Qmax)及并发症发生率。结果观察组术中出血量、尿管置留时间、手术时间、术后住院时间均较对照组低,差异有统计学意义(P<0.05);术后6个月,观察组IPSS评分较对照组低,QOL评分、Qmax水平较对照组高,差异有统计学意义(P<0.05);观察组并发症发生率较对照组低,差异有统计学意义(P<0.05)。结论良性前列腺增生采用经尿道前列腺钬激光剜除术可改善临床症状,减轻机体创伤,降低并发症发生率,改善生活质量。Objective To investigate the efficacy of transurethral holmium laser enucleation of prostate(HoLEP)in treatment of benign prostatic hyperplasia(BPH).Methods A total of 98 BPH patients in our hospital from March 2016 to September 2017 were selected,and they were randomly divided into two groups,with 49 cases in each group.Control group was given transurethral resection of prostate(TURP),while observation group was given HoLEP.After 6-month follow-up,the surgical indexes,international prostate symptom score(IPSS),quality of life(QOL),maximum flow rate(Qmax),incidence of complications were compared between the two groups.Results The intraoperative bleeding volume,urinary catheter indwelling time,operation time,postoperative hospital stays in observation group were lower than those in control group(P<0.05). After surgery for 6 months,the IPSS score of observation group was lower than that of control group,while the QOL score and Qmax level in observation group were higher than control group(P<0.05).The incidence of complications in observation group was lower than that in control group(P<0.05).Conclusion HoLEP in treatment of BPH can improve clinical symptoms,reduce trauma,decrease the incidence of complications and improve living quality.
