惠州市根除幽门螺杆菌门诊处方大数据分析

摘　　要：目的通过对惠州市阳光用药监察平台系统的8家市、县级医院2010年至2016年的根除幽门螺杆菌(Helicobacter pylori,Hp)门诊处方大数据分析,了解近7年惠州市根除Hp处方的临床应用现况,探讨不规范根除Hp处方的影响因素,减少Hp相关疾病的发生,减少Hp的耐药率,为规范根除Hp治疗提供循证学依据,为政府卫生部门对处方监督提供依据。方法利用惠州市阳光用药监察平台系统,以诊断包含"幽门螺杆菌感染"和治疗药物包含PPI或铋剂或指定抗生素,筛选出惠州市8家医院的根除Hp有效门诊处方,共23604份。分析不同等级医院、不同年份根除Hp处方的临床应用现况,并比较不同等级医院、年份根除Hp处方合格率,分析不规范根除Hp处方的原因。结果 (1)惠州市2010年至2016年根除Hp门诊处方共23604份,其中不合格处方16064份,合格处方7540份,合格处方占31.94%。(2)惠州市根除Hp门诊处方合格率从2010年24.14%上升到2016年的51.41%。2016年,"三联"处方率已经达到了35.88%,"四联"处方率达到15.53%。(3)惠州市2010年至2016年根除Hp门诊处方中,市级医院的合格率显著高于县级医院的合格率(分别为33.59%和25.29%)。结论 (1)惠州市2010年至2016年根除Hp门诊处方合格率总体上比较低,但是合格率逐年上升。卫生部门需要加强对处方的监督及医务人员的培训。(2)市级医院根除Hp治疗合格率明显高于县级医院,医生(尤其是基层医生)要及时更新知识,按指南用药,规范治疗。Objective By means of analyzing anti helicobacter pylori prescription among the eight hospitals with secondary and tertiary rank from Huizhou sunshine medical monitoring platform system during 2010-2016,to understand Huizhou clinical application status of anti helicobacter pylori prescriptionnearly seven years,explore the influence factors of non-standard anti Hp prescription,reduce the occurrence of Hp related diseases,reduce the drug resistance rate of Hp,provide a basis for evidence-based study in standard anti Hp treatment,provide the basis for government health department to supervise the prescription.Methods A total of 23604 copies anti helicobacter pylori prescriptions were selected from the 8 hospitals in Huizhou sunshine medical monitoring platform system,with the keywords of"helicobacter pylori infection"in diagnosis and"PPI or bismuth agent or designated antibiotics"contained in treatment drugs.Clinical application status of Hp prescription between different levels hospitals,different years was analyzed,and the qualified rate of anti Hp prescription between different levels hospitals,different years was compared,the causes of non-standard anti Hp prescription were analyzed.Results(1)A total of 23604 anti helicobacter pylori prescriptions were in Huizhou during 2010-2016,with 16064 unqualified prescriptions,and 7540 qualified prescriptions,qualified prescriptions accounted for31.94%.(2)Huizhou anti Hp prescription qualified rate rose from 24.14%in 2010 to 51.41%in2016.In 2016,the"municipal therapy"prescription rate had reached 35.88%,"county therapy"prescription rate reached 15.53%.(3)In the anti Hp prescriptions,the qualification rate of municipal hospitals was significantly higher than that of the county hospitals qualified rate(33.59%and 25.29%,respectively)from 2010 to 2016 in Huizhou.Conclusion(1)The qualified rate of anti Hp prescription on the overall is very low in Huizhou during 2010-2016,but the qualified rate is rising year by year.The health department needs to strengthen the supervision of
