活血法辨治慢性肾炎顽固性蛋白尿探讨

摘　　要：慢性肾炎多为本虚标实、虚实夹杂之证,其病变以脾肾亏虚为本,标实则以湿、热、瘀多见。而顽固性蛋白尿的病机往往主要表现为脾肾亏虚兼有瘀血内阻,故在补益脾肾的同时重用活血之法是巩固疗效、渐消蛋白尿的要法,一般长达3～6月。据辨证采用清热利湿活血、祛风活血、温阳活血、滋阴活血等法。强调即便是瘀血之征不显,也可因其病机而用之,不可拘泥瘀血征象。Chronic nephritis is commonly syndrome of deficiency in origin and excess in superficiality as well as deficiency-excess inclusion.Its lesion is based on spleen and kidney deficiency,while the excess in superficiality is more commonly dampness,heat and blood stasis.But the pathogenesis of intractable protein urine manifests commonly mainly as spleen and kidney deficiency with internal obstruction by blood stasis.Therefore,it is an important method of consolidating effect and gradually eliminating protein urine to greatly use the method of activating blood circulation at the same time of invigorating spleen and kidney,which is generally for up to3-6months.Clearing away heat and damp elimination with activating blood circulation,winddispersing with activating blood circulation,warming yang with activating blood circulation and nourishing yin with activating blood circulation etc.methods are used based on syndrome differentiation.It should be emphasized that even if the symptoms of blood stasis are not obvious,the above methods can also be used because of the pathogenesis,which can not rigidly stick the symptome of blood stasis.
