扶正消瘤方联合高强度聚焦超声治疗子宫肌瘤疗效观察

摘　　要：目的:观察扶正消瘤方联合高强度聚焦超声(HIFU)治疗子宫肌瘤的疗效。方法:将120例子宫肌瘤患者随机分成两组各60例,对照组给予HIFU治疗,治疗组给予扶正消瘤方联合HIFU治疗,疗程3个月。观察两组在治疗后1、3、6、12个月时子宫肌瘤体积、Adler血流分级的变化;观察治疗相关并发症及其严重程度和转归,评价疗效。结果:治疗后1、3、6、12个月,治疗组子宫肌瘤体积缩小率分别为(45.34±7.52)%、(68.91±10.15)%、(75.52±7.48)%、(83.41±5.65)%,对照组分别为(30.46±6.93)%、(51.72±8.49)%、(58.43±7.23)%,(63.82±5.34)%,组间比较差异有统计学意义(P<0.05)。治疗后第1、3个月,两组子宫肌瘤Adler血流分级比较无差异(P﹥0.05);部分肌瘤周边及肌瘤内在治疗后3个月开始出现少许点状、短条状血流信号。治疗后6、12个月,组内血流分级基本无变化,治疗组血流分级降低均高于对照组,差异有统计学意义(P<0.05)。两组均未出现严重不良反应。结论:扶正消瘤方联合HIFU治疗能促进治疗后坏死肌瘤组织吸收,抑制子宫肌瘤周边及肌瘤内血管形成。Objective:To observe the clinical effect of Fuzheng xiaoliu prescription with high intensity focused ultrasound(HIFU)on hysteromyoma.Methods:120cases of patients with hysteromyoma were randomly divided into two groups,60cases in each group.The control group was treated with HIFU,while the treatment group was treated with Fuzheng xiaoliu prescription combined with HIFU.The course of treatment was3months.The volume of hysteromyoma and the change of Adler grade of blood flow were observed in the1st,3rd,6th,12th month after treatment in two groups.The related complications and those severity as well as conversion of treatment were observed,and the effect was evaluated.Results:In the1st,3rd,6th,12th month after treatment in treatment group,the volumetric shrinkage of hysteromyoma was respectively(45.34±7.52)%,(68.91±10.15)%,(75.52±7.48)%and(83.41±5.65)%,while in control group respectively(30.46±6.93)%,(51.72±8.49)%,(58.43±7.23)%and(63.82±5.34)%,and the difference compared between two groups was statistically significant(P<0.05).In the1st and3rd month after treatment in two groups,the comparison of Adler grade of blood flow of hysteromyoma was undifferentiated(P>0.05);in the3rd month after treatment,the periphery and interior of partial hysteromyomas began to appear a little blood flow signals of point-like and strip-shaped.In the6th and12th month after treatment,Adler grade of blood flow in the groups was not changed basically;the degree of reduction of Adler grade of blood flow in treatment group was higher than in control group,and the difference was statistically significant(P<0.05).No serious adverse reactions occurred in both groups,and no hospitalization was required.Conclusion:Fuzheng xiaoliu prescription with HIFU can promote the absorption of necrotic tissue of hysteromyoma,inhibit the angiogenesis of periphery and interior of hysteromyoma and can be selected as a noninvasive treatment of hysteromyoma.
