腰椎斜扳法治疗L_5/S_1椎间盘突出症失败原因分析及应对措施

摘　　要：[目的]观察经腰椎斜板法治疗后效果不佳患者改用下肢后伸定点按压法的临床疗效。[方法]将56例入选患者采用腰椎斜板法治疗,观察疗效并探讨疗效与年龄性别病程的相关性,后将治疗失败患者43例改用肢后伸定点按压法,治疗后观察疗效。[结果]采用腰椎斜扳法治疗时,56例患者有效率约23.21%,其与年龄性别病程无相关性,43例疗效不佳患者再经下肢后伸定点按压法治疗后约79.07%患者有效,其中尤以左后、右后和极外侧突出类型的患者改善明显。[结论]下肢后伸定点按压法治疗L_5/S_1腰椎间盘突出具较好的临床疗效,未来可借助生物力学以及影像学方面探究下肢后伸定点按压法治疗L_5/S_1腰椎间盘突出症治疗机理。[Objective]To observe the clinical effect of the patients treating by the point-pressing manipulation with poor clinic effect after treatment by lumbar mitral method.[Methods]The56enrolled patients were treated with lumbar mitral method to observe the clinical effect and explore the correlation between the clinical effect and the age and gender of patient,and process of disease,then the patients who failed in the treatment were replaced with the point-pressing manipulation,and the clinical effect was observed after the treatment.[Results]When lumbar oblique manipulation was applied,56patients had an effective rate of23.21%,which was not correlated with the the age and gender of patient,and process of disease.43patients with poor clinical effect were treated with the point-pressing manipulation,and about79.07%of the patients were effective,especially the patients with left posterior,right posterior and far lateral protrusion types were significantly improved.[Conclusion]The point-pressing manipulation has a good clinical effect on L5/S1lumbar intervertebral disc herniation,and its treatment mechanism can be explored by means of biomechanics and imaging in the future.
