摘 要:目的了解adeFGH系统在鲍曼不动杆菌的多重耐药中发挥的作用。方法收集20株鲍曼不动杆菌耐药株(含10株多重耐药株),采用实时荧光定量PCR检测adeFGH外排系统编码基因adeF、adeG和adeH的mRNA表达水平,结合其耐药谱,分析adeF、adeG和adeH的mRNA表达与鲍曼不动杆菌耐药之间的关系。结果 A、B和C组鲍曼不动杆菌的adeF基因和adeG基因的mRNA的表达水平在各组之间无明显差异(P>0.05);adeH基因的mRNA表达水平在A组和B组之间无明显差异,A组和B组的adeH基因的mRNA表达水平明显高于C组(P<0.05)。结论 adeF、adeG、adeH基因共表达时,鲍曼不动杆菌adeFGH外排系统才能形成有效的抗生素外排通道;AdeFGH外排系统针对的外排底物有其局限性,主要针对氟喹诺酮类、β-内酰胺类以及β-内酰胺酶抑制剂等药物。Objective To understand the role of the adeFGH system in multi-drug resistance of Acinetobacter baumannii.Methods A total of20Acinetobacter baumannii resistant strains(including10multi-drug resistant strains)were collected.Real-time quantitative PCR was used to detect the mRNA expression levels of the encoding genes of adeF,adeG and adeH in adeFGH efflux system.The relationship between mRNA expression of adeF,adeG and adeH and Acinetobacter baumannii resistance was analyzed in combination with its drug resistance spectrum.Results The mRNA expression levels of adeF gene and adeG gene of Acinetobacter baumannii in group A,B and C were not significantly different among groups(P>0.05);the mRNA expression level of adeH gene was not significantly different between group A and group B.The mRNA expression levels of adeH gene in group A and group B were significantly higher than those in group C(P<0.05).Conclusion When the adeF,adeG and adeH genes are co-expressed,the adeFGH efflux system of Acinetobacter baumannii can form an effective antibiotic efflux channel;the adeFGH efflux system has its limitations for efflux substrates,mainly for fluoroquinolones,β-lactams,andβ-lactamase inhibitors.
