摘 要:目的:观察急性心肌梗死(AMI)患者外周血中miRNA-21、转化生长因子β1 (TGF-β1)、CC类趋化因子配体5 (CCL5)的水平,探讨其相互之间的关系及临床意义。方法:选择本院收治的AMI患者57例(AMI组)作为研究对象,另选择同时期在本院体检的健康人群50例(健康对照组),检测所有研究对象外周血中miRNA-21的表达及CCL5、TGF-β1、肌酸激酶(CK)、肌酸肌酶-同工酶(CK-MB)、心肌肌钙蛋白(cTnI)水平、冠脉Gensini积分,并分析AMI患者上述指标间的相互间关系。结果:与健康对照组比较,AMI组患者miRNA-21表达[(0.67±0.06)比(0.89±0.09)]、CCL5 [(17.53±3.75)ng/ml比(35.37±5.06)ng/ml]、CK [(85.24±18.33)U/L比(173.29±32.56)U/L]、CK-MB [(16.37±2.03)U/L比(23.34±2.51)U/L]、cTnI [(11.25±3.23)ng/ml比(31.45±8.79)ng/ml]水平及Gensini积分[(1.65±0.26)分比(43.25±12.13)分]均明显升高,TGF-β1[(31.45±8.79)ng/ml比(11.25±3.23)ng/ml]水平明显降低(P均=0.001);AMI组患者外周血中miRNA-21的表达与CCL5、CK、CK-MB、cTnI水平呈显著正相关(r=0.476~0.593,P均<0.05),与TGF-β1、冠脉Gensini积分呈显著负相关(r=-0.417、-0.625,P均<0.05)。结论:急性心肌梗死患者外周血miRNA-21呈高表达,且与CCL5、TGF-β1及冠状动脉狭窄程度密切相关,可作为AMI临床诊治的可能靶点。Objective:To observe levels of peripheral blood miRNA-21,transforming growth factorβ1(TGF-β1)and CC type chemokine ligand5(CCL5)in patients with acute myocardial infarction(AMI),and explore their relationship and clinical significance.Methods:A total of57AMI patients treated in our hospital were selected as AMI group.Another50healthy people undergoing physical examination in our hospital simultaneously were selected as healthy control group.Peripheral blood miRNA-21expression,levels of CCL5,TGF-β1,creatine kinase(CK),CK isoenzyme MB(CK-MB)and cardiac troponin I(cTnI),and coronary Gensini score were measured in all subjects.Relationship among above indexes was analyzed in AMI patients.Results:Compared with healthy control group,there were significant rise in miRNA-21expression[(0.67±0.06)vs.(0.89±0.09)],levels of CCL5[(17.53±3.75)ng/ml vs.(35.37±5.06)ng/ml],CK[(85.24±18.33)U/L vs.(173.29±32.56)U/L],CK-MB[(16.37±2.03)U/L vs.(23.34±2.51)U/L],cTnI[(11.25±3.23)ng/ml vs.(31.45±8.79)ng/ml]and Gensini score[(1.65±0.26)scores vs.(43.25±12.13)scores],and significant reduction in TGF-β1level[(31.45±8.79)ng/ml vs.(11.25±3.23)ng/ml]in AMI group,P=0.001all;in AMI group,peripheral blood miRNA-21expression was significant positively correlated with levels of CCL5,CK,CK-MB and cTnI(r=0.476~0.593,P<0.05all),and significant inversely correlated with TGF-β1level and coronary Gensini score(r=-0.417,-0.625,P<0.05both).Conclusion:Peripheral blood miRNA-21expression is high,and closely associated with levels of CCL5,TGF-β1and coronary stenotic degree in AMI patients.It may be used as a potential target for clinical diagnosis and treatment of AMI.
