糖尿病性视网膜病变硬性渗出与血脂异常关系的研究

摘　　要：目的明确硬性渗出和患者血脂水平各参数之间的关系,明确血脂指标异常是否是硬性渗出的危险因素。方法检索本院糖尿病眼病专诊数据库患者,包括患者姓名、年龄、性别、糖尿病视网膜病变诊断信息、血脂指标和眼底照片等资料。自2016年6月至2017年6月,共有74例患者入选,患者至少有一眼诊断为重度非增殖期,以渗出较重眼为入组眼。按照患者眼底彩色照片的情况,对入选患者按照硬性渗出严重程度分A、B、C 3组,统计各组患者的血清总胆固醇、甘油三酯、低密度脂蛋白指标水平的平均值。结果 A组(无明显硬性渗出组)血清总胆固醇、甘油三酯、低密度脂蛋白浓度为别为:(4.87±1.26)mmol/L、(2.65±3.70)mmol/L、(2.16±0.57)mmol/L。B组(轻度硬性渗出组):(5.15±1.27)mmol/L、(2.73±1.39)mmol/L、(2.38±0.87)mmol/L。C组(明显硬性渗出组):(6.17±1.60)mmol/L、(2.93±1.38)mmol/L、(3.04±0.90)mmol/L。3组均值比较,血清总胆固醇、低密度脂蛋白差异有统计学意义,而甘油三酯指标3组之间比较差异无统计学意义。结论患者血清总胆固醇和低密度脂蛋白异常与糖尿病视网膜病变硬性渗出严重程度相关。Objective To clarify the relationship between the parameters of blood lipid level,and to determine whether abnormal blood lipid level is a risk factor for hard exudates.Methods Searching the information on patient's name,age,gender,diagnosis of diabetic retinopathy,blood lipid level,and fundus photographs from the database of the department of diabetic eye diseases.All74patients selected from June2016to June2017.At least one eye of each patient was diagnosed with a severe non-proliferation DR,and the heavier exudate eyes were selected into the group.According to the severity of the HE by the color fundus photos of the patients,the selected patients were divided into groups A,B and C,the average of the index of serum total cholesterol(TC),triglycerides(TG),and low density lipoprotein(LDL)were measured in all groups.Results The average level of TC、TG and LDL in Group A(No obvious HE)is(4.87±1.26)mmol/L,(2.65±3.70)mmol/L,(2.16±0.57)mmol/L.In Group B(mild HE)is(5.15±1.27)mmol/L,(2.73±1.39)mmol/L,(2.38±0.87)mmol/L.In Group C(serious HE)is(6.17±1.60)mmol/L,(2.93±1.38)mmol/L,(3.04±0.90)mmol/L.The level of TC and LDL had significant difference between the three groups,but the level of TG had no significant difference in the three groups.Conclusion The extent of hard exudates in DR of type2diabetic patients with severe non-proliferative diabetic retinopathy is related with levels of TC and LDL.
