补肾化瘀法治疗宫腔粘连术后及其对局部TGF-β1和CTGF表达的影响

摘　　要：目的观察总结补肾化瘀法用于宫腔粘连术后治疗的临床疗效,并探讨其对子宫内膜转化生长因子-β1 (TGF-β1)和结缔组织生长因子(CTGF)水平的影响。方法将120例符合病例纳入标准的患者按1∶1∶1的比例随机分为试验组、对照A组、对照B组,每组各40例。3组患者均行宫腔粘连分离术。术后治疗:试验组予补肾化瘀方口服;对照A组予雌孕激素序贯治疗;对照B组宫内放置节育环治疗。疗程均为3个月经周期。结果总体疗效及中医证候评分试验组明显优于对照组;3组治疗后子宫内膜TGF-β1和CTGF的表达均较治疗前下降,试验组下降明显优于对照组。结论补肾化瘀法用于宫腔粘连术后患者的治疗效果显著,其作用机理与子宫内膜TGF-β1和CTGF水平表达的降低相关。Objective To observe the effect of using nourishing kidney and dissipating blood stasis to treat intrauterine adhesions and to discuss the expression of TGF-β1 and CTGF of using this method.Methods A total of 120 patients were randomly selected and divided into one experimental group and two control groups,each group has 40 patients.They are all treated by transcervical resection of adhesion.After surgery,the experimental group is treated by using herbal medicine to nourish kidney and dissipate blood stasis while one control group is treated by using estrogen-progestin and the other control group is treated by IUD.They are all treated for 3 periods.Results The experimental group is better than that of the control group with a statistically significant difference in total efficiency and the influence on the expression of TGF-β1 and CTGF.Conclusion The effect of using nourishing kidney and dissipating blood stasis to treat intrauterine adhesions is remarkable.The mechanism is related to the decrease of the expression of TGF-β1 and CTGF in the endometrium.
