不同剂量利伐沙班对老年心房纤颤导管消融术后抗凝治疗效果及脑卒中的预防作用

摘　　要：目的探讨不同剂量利伐沙班对老年心房纤颤导管消融术后抗凝治疗效果及脑卒中的预防作用。方法选择我院2015年7月～2018年1月收治的77例老年心房纤颤导管消融术后患者为研究对象,以随机数字表法分为A组38例与B组39例,A组术后给予7.5mg利伐沙班治疗,B组术后给予20mg利伐沙班治疗,对两组患者抗凝治疗效果及脑卒中发生率、不良反应发生率进行观察。结果两组患者术前的PT、TT、APTT、D-D等凝血功能指标比较,差异无统计学意义(P> 0.05),术后3个月时,两组患者凝血功能指标均有改善,差异有统计学意义(P <0.05),但两组凝血功能比较,差异无统计学意义(P> 0.05); A组术后无脑卒中发生,与B组10.26%相比,差异有统计学意义(P <0.05);A组不良反应发生率为7.89%,较B组30.77%低,差异有统计学意义(P <0.05)。结论老年心房纤颤导管消融术后应用小剂量利伐沙班抗凝治疗即可取得良好抗凝效果,可预防脑卒中发生,也能减少不良反应发生率,值得推广。Objective To explore the effects of different doses of rivaroxaban on anticoagulant therapy and stroke prevention after catheter ablation in elderly patients with atrial fibrillation.Methods77elderly patients with atrial fibrillation who underwent catheter ablation from July2015to January2018were selected as subjects and randomly divided into group A(n=38)and group B(n=39).Group A was treated with rivaroxaban7.5mg and group B was treated with rivaroxaban20mg.The anticoagulant effect,incidence of stroke and incidence of adverse reactions in the two groups were observed.Results There was no significant difference in coagulation function such as PT,TT,APTT and D-D between the two groups(P>0.05).At3months after operation,the coagulation function of the two groups were improved,and the difference was statistically significant(P<0.05),but there was no significant difference between the two groups(P>0.05).There was no stroke in group A after operation,which was significantly different from that in group B(10.26%),The difference was statistically significant(P<0.05).The incidence of adverse reactions in group A was7.89%,which was lower than that in group B(30.77%),and the difference was statistically significant(P<0.05).Conclusion After atrial fibrillation catheter ablation,the use of small dose of rivaroxaban anticoagulant therapy in elderly patients can achieve good anticoagulant effect,can prevent stroke,but also reduce the incidence of adverse reactions,worthy of promotion.
