回收式自身输血在中等量出血手术中的应用价值

摘　　要：目的探讨回收式自身输血技术在中等量出血的手术中对减少异体输血的临床应用价值。方法收集我院2016年1月～2017年12开展回收式自身输血技术前后两年中206例中等出血量(出血量在500～1500mL)手术患者,按照回收式自身输血技术开展前后分组,以其中2016年94例应用异体输血手术患者为对照组,以2017年112例应用回收式自身输血手术患者为实验组,观察他们手术前后(24小时内)的Hb和Hct变化情况,并比较两组间的异体输血率和平均输血量。结果对照组与实验组术后Hb和Hct数据比较P> 0.05,无明显差异;实验组异体输血率为59.57%,平均输血量为2.24U,对照组异体输血率为16.07%,平均输血量为0.44U,实验组异体输血率和平均输血量明显高于对照组,两组间比较差异具有统计学意义(P <0.001)。结论对于中等出血量的手术患者采用同种异体输血或回收式自身输血都能达到相同的治疗效果,而且回收式自身输血技术能有效减少异体输血,达到节约血液资源的目的。Objective To explore the clinical application value of salvaged blood autotransfusion in the moderate hemorrhage surgery.Methods206cases with moderate hemorrhage surgery(the amout of bleeding in500~1500ml)from January2016to December2017were collected and grouped by before and after the implementation of salvaged blood autotransfusion technology.In2016,94patients with allogeneic blood transfusion surgery were selected as the control group.In2017,112patients with salvaged blood autotransfusion surgery were selected as the experimental group.The changes of HB and Hct before and after operation(within24hours)were observed,the allogeneic transfusion rate and average transfusion volume were compared between the two groups.Results There was no significant difference in Hb and Hct data between the control group and the experimental group(P>0.05).The allogeneic transfusion rate was59.57%in the experimental group and16.07%in the control group,the average transfusion volume was2.24U in the experimental group and0.44U in the control group.The allogeneic transfusion rate and average transfusion volume in the experimental group were significantly higher than the control group,there was significant difference between the two groups(P<0.001).Conclusion The patients with moderate hemorrhage surgery can achieve the same therapeutic effect by allogeneic blood transfusion or salvaged blood autotransfusion,and salvaged blood autotransfusion technology can effectively reduce allogeneic blood transfusion and save blood resources.
