床旁血气分析仪血乳酸血细胞压积与血色素快速检测在休克容量复苏中的使用价值

摘　　要：目的探讨使用床旁血气分析仪血乳酸血细胞压积与血色素快速检测应用于休克容量复苏抢救中的效果。方法回顾性分析本院手术室2016年6月～2017年6月出现休克重症患者的复苏抢救过程,同时在复苏抢救中应用了床旁血气分析仪血乳酸血细胞压积与血色素快速检测的方法。结果应用分析仪快速检测患者血乳酸,显示患者的血乳酸值同患者血色素、心率及全血乳酸呈现出正相关的关系,且在复苏前与复苏后不同的时间,相关指标间的水平具有较为显著的差异(P <0.05);经快速检测后,100例休克患者院内转运及CT检查成功率为81.00%,存活率为96.00%。结论针对手术室的休克患者,应用床旁血气分析仪血乳酸血细胞压积与血色素快速检测可迅速获得患者血乳酸值,这样为抢救医师评估患者容量复苏效果提供参考,进而纠正患者休克情况,改善患者预后。Objective To explore the effect of rapid detection of blood lactate hematocrit and hemoglobin by bedside blood gas analyzer in rescue of shock volume resuscitation.Methods The resuscitation processes of severe shock patients in our operating room from June2016to June2017were retrospectively analyzed,at the same time,the bedside blood gas analyzer was used in resuscitation rescue to rapidly detect blood lactate hematocrit and hemoglobin.Results Rapid detection of serum lactic acid by analyzer showed that serum lactic acid was positively correlated with hemoglobin,heart rate and whole blood lactic acid.At different time before and after resuscitation,the levels of related indicators were significantly different(P<0.05).After rapid detection,the success rate of hospital transportation and CT examination was81.00%and the survival rate was96.00%of100shock patients.Conclusion For shock patients in operating room,rapid detection of blood lactate hematocrit and hemoglobin by bedside blood gas analyzer can quickly obtain blood lactic acid value,which can provide reference for rescue doctors to evaluate the effect of volume resuscitation,correct the shock situation of patients and improve the prognosis of patients.
