摘 要:目的 掌握湖北省潜在流行区血吸虫病流行动态,及时发现血吸虫病传播风险因素,为推进防治工作提供科学依据和技术保障。方法 2015—2017年,在湖北省潜在流行区,包括十堰市、宜昌市、随州市、恩施州和神农架林区等5个市(州、林区),开展人群、家畜和螺情监测,结合当地自然因素,应用风险矩阵对潜在流行区血吸虫病传播风险进行评估。结果 2015—2017年,在5个市(州、林区)开展了血吸虫病监测。对16671人进行了血清学检测,其中包括从业人员12917人,重点人群505人,流动人群3249人。血清学检测阳性6人,阴性16665人。共检测家畜791头,其中本地家畜212头,引进家畜579头,未发现阳性家畜。进行了钉螺输入风险监测、漂浮物监测和可疑环境调查,均未发现钉螺。综合评估提示,湖北省潜在流行区血吸虫病传播风险等级为低风险。结论 在潜在流行区开展血吸虫病监测和评估有重要意义,要继续加强疫情监测,防止传染源、钉螺输入,建立长期、有效、系统的监测预警体系,降低潜在流行区血吸虫病传播风险。Objective To grasp the riskof schistosomiasis epidemiologicalin potential endemic areas in Hubei Province,so as to provide the evidence for promoting the prevention and control work. Methods From 2015 to 2017,Schistosomiasis infection in population and livestock,the distribution of Oncomelania hupensis were monitored in potential endemic areas in Hubei Province,including Yichang City,Shiyan City,Enshi Prefecture,Suizhou City,Shennongjia Forest Area.The risk of transmission of schistosomiasis in potential endemic areas was assessed by risk matrix combining with local natural factors. Results From 2015 to 2017,schistosomiasis surveillance was carried out in five cities(states,forests).There were 16 671 people were examined by Serological tests,including 12 917 practitioners,505 high risk populations,and 3 249 migrants.Serological tests found that 6 peoplewere positive and 16 665 people were negative.A total of 791 livestock were tested,including 212 domestic animals and 579 domestic animals.No positive livestock were found.The snail input risk monitoring,floating object monitoring and suspicious environmental investigations were conducted,and no snails were found.The comprehensive assessment suggests that the risk level of schistosomiasis transmission is low in potential endemic areas in Hubei Province. Conclusion It is significance to carry out schistosomiasis monitoring and assessment in potentially endemic areas.In order to prevent infection sources and snail input,it is necessary to continue to strengthen surveillance.A long-term,effective and systematic monitoring and early warning system should be established to reduce the risk of transmission of schistosomiasis in potential endemic areas.
