体质指数对急性冠脉综合征病人血压的影响

摘　　要：目的探讨体质指数(BMI)对急性冠脉综合征(ACS)病人血压的影响。方法选取2014年12月—2016年3月我院收治的ACS病人109例,根据是否合并高血压分为高血压组(77例)和非高血压组(32例),收集病人一般资料,采用单因素分析和多因素分层回归分析,观察BMI对ACS病人血压的影响。结果高血压组BMI值明显高于非高血压组(P <0.01)。单因素分析结果显示,BMI对收缩压(SBP)、舒张压(DBP)均未见显著影响;多元线性回归分析结果显示,调整了脑卒中病史、吸烟史、用药史等混杂因素的影响,根据年龄和吸烟情况进行分层分析,年龄、吸烟和BMI对DBP产生显著交互作用(P <0.01),低年龄段ACS病人BMI与DBP显著相关(P <0.01)。结论控制体重对中青年人群高血压防控,特别是合并ACS病人舒张压的降低有重要意义。Objective To analyze the effect of body mass index(BMI)on blood pressure in patients with acute coronary syndrome(ACS).Methods One hundred and nine patients with ACS admitted in our hospital from December2014to March2016were randomly divided into hypertension and non hypertension group according to whether the patients accompanied by hypertension.Clinical data were collected and analyzed from the patients′medical records,included general information.All data were analyzed using single factor analysis and multifactor hierarchical regression analysis to observe the influence of BMI on blood pressure in patients.Results The BMI in hypertension group was obviously higher than that in non hypertension group(P<0.01).The result of univariate analysis showed BMI had no significant effect on systolic blood pressure(SBP)and diastolic blood pressure(DBP).Multiple linear regression analysis showed when the effects of confounding factors such as the stroke history,smocking history and medication were adjusted,age,smoking,and BMI had significant interaction with DBP(P<0.01)and BMI of ACS patients in low age group was significantly correlated with DBP(P<0.01),according to the age and smoking status hierarchical analysis.Conclusion Weight control is a great method for the prevention and control of hypertension in young and middle aged people,especially lower diastolic blood pressure in patients with ACS.
