实时三维盆底超声对温针灸联合盆底肌锻炼治疗女性压力性尿失禁的疗效评估

摘　　要：目的探讨实时三维盆底超声观察温针灸联合盆底肌锻炼治疗女性压力性尿失禁(SUI)的疗效。方法筛选70例符合条件的SUI患者,随机分成对照组和试验组各35例,其中对照组采用单纯盆底肌锻炼,试验组采用温针灸联合盆底肌锻炼。1个月后行疗效评估及经会阴实时三维盆底超声检查,比较两组治疗前后静息和Valsalva状态下肛提肌厚度(MLT)、肛提肌裂孔左右径(LHW)和面积(LHA),以及Valsalva状态下膀胱颈移动度(BLM)。结果两组经治疗后临床症状均改善,试验组更为显著,差异有统计学意义(P<0.05);两组经治疗后静息和Valsalva状态下MLT增厚、LHW、LHA和BLM均减小,其中试验组更显著,差异均有统计学意义(均P<0.05)。结论温针灸联合盆底肌锻炼可有效提高单纯盆底锻炼治疗SUI的疗效;实时三维盆底超声可客观评估盆底相关结构的变化。Objective To explore the curative effect of the combination of warm acupuncture and pelvic floor muscle exercise in female stress urinary incontinence(SUI)by real-time three-dimensional pelvic floor ultrasound.Methods Seventy patients with eligible SUI were screened and randomly divided into control group and experimental group,35 cases in each group.The control group was treated with the simple pelvic floor muscle exercise,while the experimental group was treated with the warm acupuncture combined with pelvic floor muscle.After 1 month,the efficacy was evaluated and the real-time three-dimensional pelvic floor ultrasonography was performed.The levels of levator ani muscle thickness(MLT),the levator ani muscle hole diameter(LHW)and area(LHA),and bladder neck mobility(BLM)of the two groups before and after treatment were compared at rest and Valsalva state.Results The clinical symptoms of both groups were improved after treatment,and the experimental group was more significant(P<0.05).After treatment,the MLT,LHW,LHA and BLM were decreased at rest and Valsalva state,and the experimental group were more significant with statistical difference(all P<0.05).Conclusion The combination of warm acupuncture and pelvic floor exercise is more effective in improving the curative effect of pure pelvic floor exercise in treating SUI.Real-time three-dimensional pelvic floor ultrasound can objectively evaluate the changes of pelvic floor related structure.
