摘 要:目的 通过调查轻度烧伤患者全身性使用抗菌药物后创面病原微生物的分布及耐药情况,判断全身性使用抗菌药物是否能有效预防创面感染,提出正确应对的方案。方法 本研究回顾2014年1月至2017年12月,四年间本科门诊烧伤患者,根据有无全身性使用抗菌药物的情况,分为抗菌药物治疗组及非抗菌药物治疗组,创面分泌物细菌培养使用法国生物梅里埃公司ATB Expression半自动细菌鉴定仪,按照《全国临床检验操作规程》进行细菌鉴定,所有数据采用SPSS19.0软件进行统计分析。结果 抗菌药物使用组与非抗菌药物使用组创面细菌检出率无明显差异,革兰氏阳性菌前组占比83.1%,后者占比86.4%,两组比较差异无统计学意义(P>0.05);抗菌药物使用组创面革兰氏阳性菌耐药菌株较非抗菌药物使用组明显增高(P>0.05),两组耐药菌株比较差异无统计学意义(P>0.05)。结论 轻度烧伤患者全身性使用抗菌药物不能降低创面感染概率,反而促进了革兰氏阳性菌耐药菌株的产生。Objective To investigate the distribution and drug resistance of pathogen isolated from mild degree burn patients after systemic antibacterial therapy.Methods From January 2014 to December 2017,outpatients with mild degree burn in our hospital were selected,and were divided into two groups according to whether accepting systemic antibacterial therapy or not.The pathogens cultured from the wound of the burn patients were identified by the ATB Expression semi-automatic bacteria identification system of Biomerieux France.The bacterial identification was carried out according to National Clinical Laboratory Operation Guidelines.All of the data were statistically analyzed by the SPSS19.0 software.Results There was no statistically significant difference in the rate of appearance of pathogens between the two groups.The percentage of gram-positive was 83.1%in systemic antibacterial therapy group,86.4%in non systemic antibacterial therapy group.The strains of drug resistant gram-positive bacteria in systemic antibacterial therapy group were more than another group,there was no statistically significant difference in drug resistant gram-negative bacteria between the two groups.Conclusion Systemic antibacterial therapy can’t lower the infection rate of burn wound in mild burn patients,but it can increase the number of strains of drug resistant gram-positive bacteria.
