摘 要:目的探讨成人先天性房缺(ASD)患者房间隔缺损口血液流速对机体循环微粒(MPs)水平及功能的影响。方法选取2016-02-01~2017-02-28之间就诊于延安大学附属医院的成人先天性ASD患者30例为实验组,对照组为期间来我院健康体检的志愿者30例。经心脏B超记录实验组缺损口血流速度;提取两组患者血浆中MPs,检测MPs含量并分析其与房间隔缺损口血流速度的相关性;用两组MPs处理离体大鼠胸主动脉检测对血管舒张功能及其相关的诱导型一氧化氮合酶(i NOS)和内皮素-1(ET-1)的影响。结果与对照组(2. 35±0. 73)mg/m L相比,成人先天性房缺组(4. 23±0. 88) mg/m L,血浆中MPs水平明显升高(P <0. 05),且MPs水平与房间隔缺损口血流速度呈正相关(r=0. 60,P=0. 0012),实验组MPs较对照组明显抑制大鼠离体胸主动脉血管舒张,刺激i NOS和ET-1表达(P <0. 05)。结论成人先天性ASD引起血流动力学的改变导致机体MPs升高,而升高的MPs可能进一步导致血流动力学的不稳定。Objective To study the effect of blood flow velocity on the circulation particle(MPs)level and function in patients with congenital atrial septal defect(ASD).Methods Adult patients(n=30)with congenital ASD who were treated in Affiliated Hospital of Yan'an University from February2016to February2017were selected as the experimental group,and the control group was30volunteers in our hospital during this period.The cardiac ultrasonography indexes of blood flow velocity in ASD were recorded,MPs from two groups was extracted by centrifugation.Both the content of MPs and its correlation with blood flow velocity of atrial septal defect orifice was analyzed.Effect of MPs on the vasodilation function,iNOS and ET-1of isolated rat thoracic aorta was detected.Results Compared with control group(2.35+0.73mg/mL),MPs in the experimental group(4.23+0.88mg/mL)was significantly increased(P<0.05).The level of MPs in experimental group was positively correlated with blood flow velocity of atrial septal defect(r=0.60,P=0.0012).MPs in the experimental group significantly inhibited the vasodilatation,increasing iNOS and ET-1expression of the thoracic aorta of the rat(P<0.05).Conclusion The changes in hemodynamics caused by congenital ASD in adults lead to the increase of MPs,and the elevated MPs may further lead to the instability of hemodynamics.
