摘　　要：目的 为了早期发现及防止发展儿童慢性肾脏疾病的发生,调查分析广州荔湾地区健康学龄前儿童的尿液异常发病率。方法 收集2013至2017年小学1年级入学前6~7岁健康学龄前儿童常规的中段晨尿,采用尿干化学试纸法及尿沉渣分析法。结果 本研究共纳入调查儿童13 992名,总的尿液异常率为13.68%(1 914例),其中男孩为6.97%(523例),女孩为21.40%(1 391例);男孩尿白细胞异常64例(0.85%)、红细胞异常438例(5.84%)、尿蛋白30例(0.40%),女孩尿白细胞异常842例(12.98%)、红细胞异常582例(8.97%)、尿蛋白39例(0.60%)。女孩尿白细胞、红细胞异常明显高于男孩,差异有统计学意义(P<0.05),男孩和女孩尿蛋白异常无明显差异。结论 广州市荔湾区学龄前儿童的尿液异常率筛查,健康儿童存在高尿液分析异常率。进行健康学龄前儿童尿筛查对小儿隐匿性泌尿系统疾病的早期发现和早期干预非常必要,及追踪是否发展儿童慢性肾脏疾病具有积极意义。Objective To early detect and prevent the development of chronic kidney disease(CKD)in children investigating and analyzing the incidence of urine abnormality in healthy preschool children at Liwan District,Guangzhou.Methods The routine samples of midstream morning urine of 6-7 years old healthy preschool children were collected from 2013 to 2017,and were analyzed by dry-chemistry test paper method and urinary sedimentanalytical method.Results 13 992 children were investigated in this study.The total abnormality rate of urine was 13.68%(1 914 cases),and was 6.97%(5 23/7 504)in the boys and 21.40%(1 391/6 488)the girls.In the boys,64 cases(0.85%)had abnormal white blood cell(WBC),438 cases(5.84%)urine red blood cell(RBC),and 30 cases(0.40%)proteinuria;and in the girls,842(12.98%),582(8.97%),and 39(0.60%).The abnormalities of urine WBC and RBC were significantly higher in the girls than in the boys(P<0.05).There was no statistical difference in proteinuria between the boys and the girls.Conclusion The abnormality rate of urine of preschool children at Liwan District,Guangzhou was screened.There is a high abnormality rate of urine analysis in healthy children.It’s very essential to have urine screening for healthy preschool children,which can not only early detect but also intervene the occult diseases of urinary system,and it is of great significance to track the development of chronic kidney disease(CKD)in children.
