摘　　要：目的 评估RP-TVT治疗女性SSUI的临床疗效。方法 回顾性收集本院泌尿外科2012年11月至2018年7月收治的有尿流动力学资料的32例女性SSUI患者的临床资料。通过分析患者手术前后ICI-Q-SF、PISQ-12、Qmax及PVR差异,评估RP-TVT手术治疗女性SSUI的疗效。结果 32例患者中有效率96.88%;术前症状评分(FOL+VOL)及QOL评分分别为(7.97±2.06)分、(7.69±1.47)分,与术后各随访时点差异均有统计学意义(均P<0.05);PISQ-12评分术前为(24.50±2.69)分,术后3个月及36个月差异均无统计学意义(均P>0.05),其余随访时点差异均有统计学意义(均P<0.05);Qmax术前与术后各随访时点差异均有统计学意义(均P<0.05),但最低时仍达到(19.50±2.54)ml/s,PRV除术后3个月时(6.25±3.81)ml,但均≤10 ml,差异均有统计学意义(均P<0.05),其余各随访时点差异均无统计学意义(均P>0.05);患者术中均无膀胱、尿道损伤。术后1例并发尿潴留,剪断吊带后排尿通畅,无漏尿;1例并发尿频、尿急,予索利那新治疗后改善,无漏尿。结论 RP-TVT治疗女性SSUI效果确切,并且安全、微创,值得临床推广使用。Objective To evaluate the efficacy and safety of retropubic tension-free vaginal tape(RPTVT)for female simple stress urinary incontinence(SSUI).Methods A retrospective study was conducted to collect the clinical data of 32 women with SSUI admitted our department from November,2012 to July,2018.By analyzing the differences of ICI-Q-SF,PISQ-12,Qmax,and PVR between before and after RP-TVT,the efficacy and safety of RP-TVT for SSUI were evaluated.Results Among the 32 patients,28 were cured,3 were improved,1 were ineffective,and the effective rate was 96.88%.The preoperative(FOL+VOL)and QOL scores were(7.97±2.06)and(7.69±1.47),respectively,which were significantly different from those at all the followup points(P<0.05).The preoperative PISQ-12 score was(24.50±2.69),which was not significantly different from those 3 and 36 months after the operation(P>0.05),but were form those at the other follow-up points(P<0.05).The preoperative Qmax was different from those at all the follow-up points(P<0.05),and its minimum value was(19.50±2.54)ml/s.3 months after the operation,the PRV was(6.25±3.81)ml,but all were less than 10 ml,which was statistically different from that before the operation(P<0.05);but there were no statistical differences between that before the operation and those at all the other follow-up points(all P>0.05).No bladder or urethra injury occurred during the operation.1 patients got urinary retention,and had no recurrence of leakage after cutting the sling.Another patient was complicated with urinary frequency and urgency,and the symptoms were improved and no recurrence of leakage occurred after treatment with solinaxine.Conclusion RP-TVT is an effective,minimally invasive,and safe treatment for female SSUI,and is worth for clinical application.
