经皮椎间孔镜技术精准减压治疗高龄退变性腰椎侧凸伴神经根病的短期疗效

摘　　要：目的探讨椎间孔镜技术精准减压治疗高龄退变性腰椎侧凸伴神经根病的短期疗效及安全性。方法选取2015-01—2017-12间在郑州市第一人民医院接受经皮椎间孔镜下神经根减压手术治疗的44名退变性腰椎侧凸合并单侧神经根刺激症状患者,回顾性分析患者的临床资料,并对疗效进行评价。结果本组患者住院时间(3. 1±1. 5) d,手术时间(64±10. 2) min,术中出血量(20±11. 7) m L。未发生明显神经根损伤、脑脊液漏、感染等术后并发症。随访期间未出现腰椎侧凸明显进展现象。术后均获随访6. 1个月,末次随访时VAS评分及ODI评分明显下降(P <0. 05)。结论经皮椎间孔镜技术治疗退变性腰椎侧凸,创伤小、短期疗效显著,但长期效果仍需进一步研究。Objective To evaluate the Short-term clinical efficacy of percutaneous endoscopic lumbar discectomy for degenerative lumbar scoliosis with radiculopathy in elderly patients. Methods Forty-four elderly patients were included from January2015to December2017,and their clinical materials were analyzed retrospectively.There were24women and20men with a mean age of72years(range,61-79years).All patients had degenerative lumbar scoliosis,associated with degenerative radiculopathy.We have made a strict screening criteria,and all the patients were treated with lateral approach.Preoperative,postoperative,and follow-up questionnaires were obtained to evaluate clinical outcomes. Results The mean operation time lasted for(64±10.2)min,and intraoperative blood loss of(20±11.7)ml.Of them,patients were followed up for a mean of6.1months.The VAS score and ODI score decreased significantly compared with preoperative at the last follow-up and Indicating a obvious relief of symptoms.More importantly,there were no obvious nerve root injury,cerebrospinal fluid leakage,infection and other complications.Besides,we have not found obvious progression of lumbar scoliosis in patients during the follow-up time. Conclusion In elderly patients with degenerative lumbar scoliosis,percutaneous endoscopic lumbar discectomy was less invasive and better results for the short term,however,further study is still needed for the long-term effect.
