福建地区孕妇TORCH感染与不良妊娠结局的相关性研究

摘　　要：目的探索福建地区孕妇TORCH感染与不良妊娠结局间的相关性,为制订有针对性的干预措施提供参考依据。方法以2016～2017年在福建省妇幼保健院接受产前检查的1 250例孕妇作为研究对象,空腹采集孕妇血液样本,采用酶联免疫法检测孕妇血清抗TORCH-IgM抗体。对抗体阳性孕妇,随访追踪其妊娠结局,比较抗TORCH-IgM抗体阳性和阴性孕妇不良妊娠结局发生率。结果 1 250例孕妇中,抗TORCH-IgM抗体总体阳性率为4. 08%。抗TOX-IgM、抗RUB-IgM、抗CMV-IgM和抗HSV-IgM抗体阳性率分别为0. 8%、0. 48%、1. 6%和1. 04%,差异有统计学意义(χ~2=8. 64,P <0. 05)。51例抗TORCH-IgM抗体阳性孕妇中,不良妊娠结局发生率为61. 76%; 1 199例抗TORCH-IgM抗体阴性孕妇中,不良妊娠结局发生率为1%。抗TORCH-IgM抗体阳性和阴性孕妇不良妊娠结局发生率差异有统计学意义(校正χ~2=291. 77,P <0. 01)。结论孕期抗TORCH-IgM抗体阳性则是引发孕妇不良妊娠结局的重要危险因素。Objective To investigate the correlation between TORCH infections during pregnancy and poor pregnant outcomes in women in Fujian area for evidence to scientifically plan targeted interventions for such syndrome.Methods 1 250 pregnant women undergone prenatal diagnosis in Fujian Provincial Maternity & Children Hospital were included from 2016 to 2017.Fast blood samples were obtained to detect the anti-TORCH IgM antibodies using an enzyme-linked immunoassay.Women with positive serum findings were followed with pregnant outcomes.Then incidence of poor pregnant outcomes was compared between women with positive anti-TORCH IgM antibodies and those with seronegative anti-TORCH IgM antibodies.Results The overall prevalence of anti-TORCH IgM antibodies was 4.08% in the study populations,and the prevalence of anti-TOX,anti-RUB,antid-CMV and anti-HSV IgM antibodies was 0.8%,0.48%,1.6% and 1.04%,respectively.The difference was significantly different(χ^2=8.64,P<0.05).Incidence of poor pregnant outcomes was 61.76% in the 51 women with seropositive anti-TORCH IgM antibodies,and 1% in the 1 199 women with seronegative anti-TORCH IgM,with significant difference between groups(adjustedχ^2=291.77,P<0.01).Conclusion TORCH infection during pregnancy is an important risk factor for poor pregnant outcomes.
