后路椎弓根螺钉内固定联合髂骨植骨术治疗老年患者AndersonⅡ型齿状突骨折

摘　　要：目的:探究后路椎弓根螺钉内固定联合髂骨植骨术治疗老年患者AndersonⅡ型齿状突骨折效果。方法:选取医院在2016年11月至2017年10月期间收治的AndersonⅡ型齿状突骨折16例老年患者,将他们随机均等分为两组,对照组采用后路椎弓根螺钉内固定治疗,研究组在对照组基础上联合髂骨植骨术治疗,观察对比两组患者手术前后颈椎日本矫形外科手术评分(JOA)、VAS疼痛评分及颈椎功能障碍指数(NDI)评分情况,且观察比较两组患者的生活质量水平。结果:治疗前,两组患者的JOA、VAS和NDI评分差异无统计学意义(P>0.05),治疗后研究组患者的VAS评分和NDI评分明显低于对照组,且JOA评分明显高于对照组(P <0.05);治疗前,两组患者的生活质量水平差异无统计学意义(P>0.05),治疗后,研究组患者的认知功能、躯体功能和角色功能和社会功能水平明显高于对照组(P<0.05)。结论:后路椎弓根螺钉内固定联合髂骨植骨术治疗老年患者AndersonⅡ型齿状突骨折不仅可以达到了良好的治疗效果,还可以改善患者的颈椎功能,提高生活质量水平,但是对于远期患者颈椎的旋转功能会产生不同程度的影响。Objective:Objective to explore the treatment of Anderson II odontoid fracture in elderly patients by posterior pedicle screw fixation combined with iliac bone graft.Methods:16cases of senile patients with Anderson II odontoidfracture treated in our hospital during the period of January-2018November2016were analyzed retrospectively.Theywere divided into two groups randomly.The control group was treated with posterior pedicle screw fixation.The study group was treated with iliac bone grafting on the basis of the control group,and the two groups were observed and com-pared.The scores of the Japanese Orthopedic Surgery score(JOA),the VAS pain score and the cervical spine dysfunc-tion index(ND1)were evaluated before and after the operation,and the quality of life of the two groups wascompared.Results:Before treatment,there was no difference in JOA,VAS and NDI scores between the two groups(P>0.05).The VAS score and the NDI score of the patients in the study group were significantly lower than those of the control group,and the JOA score was significantly higher than that of the control group(P<0.05).Before the treatment,the quality of life of the two groups had no regional difference(P>0.05).After treatment,the patients in the study group were treated.The cognitive function,physical function and role function and social function level of the control group were significantly higher than those of the control group(P<0.05).Conclusion:Posterior pedicle screw fixation combined with iliac bone grafting in the treatment of Anderson type II odontoid fracture in elderly patients can not only achieve good therapeutic effect,but also improve the function of the cervical spine and improve the quality of life,but it will have different degrees of influence on the rotation function of the long-term patients.
