人工股骨头置换术与空心加压螺钉固定治疗股骨颈骨折对比研究

摘　　要：目的探讨人工股骨头置换术与空心加压螺钉固定治疗股骨颈骨折的临床疗效。方法将2016年4月～2017年3月我院治疗的老年股骨颈骨折患者112例作为研究对象,按随机数字表法分为两组各56例。对照组行空心加压螺钉固定治疗,观察组接受人工股骨头置换术治疗,观察比较两组患者的手术情况、术后恢复指标、髋关节功能恢复情况及并发症发生率。结果观察组患者的卧床时间、住院时间均短于对照组,手术时间、术中出血量高于对照组,差异均有统计学意义(P<0.05);两组髋关节功能恢复优良率比较,观察组相对高于对照组,且并发症发生率低于对照组,差异均有统计学意义(P<0.05)。结论人工股骨头置换术是治疗老年股骨颈骨折的一种较为安全、有效的手术方式,可缩短患者卧床时间,减少术后并发症,促进髋关节功能恢复,但术中创伤大,临床应依据患者实际情况选择适宜的手术方式。Objective To compare the clinical efficacy of artificial femoral head replacement and hollow compression screw fixation in treatment of femoral neck fracture.Methods112senile patients with femoral neck fracture treated in our hospital from April2016to March2017were selected,and they were divided into two groups by random number table,with56cases in each group.Control group was given hollow compression screw fixation,while observation group was given artificial femoral head replacement.The surgery,postoperative recovery indexes,hip joint function,incidence of complications in two groups were observed.Results The bed rest time and hospital stays of observation group were less than those of control group,while the operation time and intraoperative bleeding volume in observation group were higher than those in control group(P<0.05);Compared with control group,the excellent rate of hip joint function recovery in observation group was higher,while the incidence of complications was lower(P<0.05).Conclusion Artificial femoral head replacement in treatment of femoral neck fracture is safe and effective,it can shorten bed rest time,reduce postoperative complications,promote early recovery of hip joint function,but it has larger intraoperative trauma.Therefore,it is necessary to choose suitable procedure according to patients'specific conditions.
