摘  要:目的探讨生物素化微细纤维肽修饰胰岛素样生长因子-1(IGF-1)对急性心肌梗死兔左心室重构的影响。方法将28只健康的、雄性新西兰白兔随机分为对照组和实验1组、实验2组、实验3组,每组7只。将实验1、2、3组的冠状动脉左前降支结扎来建立兔急性心肌梗死模型,对照组不结扎冠状动脉左前降支。模型建立后,实验1组注射生物素化微细纤维肽修饰的IGF-1,实验2组注射未修饰的IGF-1,实验3组和正常对照组注射0.9%氯化钠注射液。分别检查各组干预6~8h后和干预1、8周后的心电图(ECG),干预1、4、8周后左心室舒张末期内径(LVEDd)、左心室射血分数(LVEF)、心输出量(CO)的水平。结果实验3组干预8周后ECG示出现宽大的病理性Q波。实验1、2、3组干预1、4、8周后LVEDd水平均显著高于对照组,LVEF、CO水平均显著低于对照组(均P<0.05)。干预1、4、8周后,实验1、2组LVEDd水平均显著低于实验3组,且实验1组又均显著低于实验2组(均P<0.05);实验1、2组LVEF、CO水平均显著高于实验3组,且实验1组又均显著高于实验2组(均P<0.05)。结论生物素化微细纤维肽修饰IGF-1能显著改善兔急性心肌梗死后心脏的缺血及左心室重构,且有利于心肌功能的修复。Objective To investigate the effect of biotinylated microfiber peptides-modified insulin-like growth factor-1(IGF-1)on the left ventricular remodeling in rabbits with acute myocardial infarction(AMI).Methods Twenty-eight male New Zealand white rabbits were randomly divided into control group,experimental group1,experimental group2and experimental group3,with7rabbits in each group.The AMI was induced by ligating the left anterior descending coronary artery in experimental groups.The control group received sham operation only.After model establishment,the experimental group1was injected with biotinylated microfiber peptides-modified IGF-I,and the experimental group2with unmodified IGF-I.Both control group and experimental group3were given0.9%sodium chloride injection.The electrocardiogram(ECG)was observed after treatment for6-8hours and intervention for1and8weeks.Furthermore,left ventricular end-diastolic diameter(LVEDd),left ventricular ejection fraction(LVEF)and cardiac output(CO)were measured after1,4,and8weeks of intervention.Results In experimental group3,ECG still showed large pathological Q waves after8weeks of intervention.Compared with control group,the LVEDd increased and LVEF and CO decreased in all the three experimental groups after1,4and8weeks of intervention(P<0.05).Compared with group3,the LVEDd reduced and the LVEF and CO increased in both group1and group2(P<0.05).Compared with group2,the LVEDd decreased and the LVEF and CO increased in group1(P<0.05).Conclusion Biotinylated microfiber peptides-modified IGF-1can significantly improve cardiac ischemia and left ventricular remodeling and is beneficial to the repair of myocardial function in rabbits with AMI.
