当归四逆汤合一贯煎配合针刺治疗顽固性腰痛的效果

摘　　要：目的观察并比较针药联合和针刺治疗顽固性腰痛的临床疗效差异。方法选取2015年1月～2016年12月我院收治的58例顽固性腰痛患者作为研究对象,采用随机数字表法将其分为针药组(30例)、针刺组(28例)。两组患者针刺治疗时选取的穴位相同,主穴均为十七椎、腰阳关、肾腧、大肠腧、环跳、阳陵泉。针药组患者采用当归四逆汤合一贯煎加减配合针刺,针刺组患者仅采用普通针刺。两组患者针刺治疗均隔日治疗1次,针药组中药汤剂每日1剂。4周为1个疗程,1个疗程后,比较两组患者的视觉模拟量表(VAS)评分和临床疗效。结果治疗后,针药组中的总有效率为86.7%,针刺组的总有效率为71.4%,针药组患者的总有效率高于针刺组,差异有统计学意义(P<0.05);针药组患者治疗后的VAS评分低于针刺组,差异有统计学意义(P<0.05)。结论当归四逆汤合一贯煎加减配合针刺方法对顽固性腰痛的疗效优于普通针刺方法。Objective To observe and compare the clinical efficacy of acupunctural therapy combined with Chinese herbal medicine versus acupunctural therapy alone in the treatment of refractory low back pain.Methods From January 2015 to December 2016,58 patients with refractory low back pain were selected as subjects and were divided into acupuncture+herbal medicine group(n=30)and acupuncture group(n=28).The acupuncture points selected by the two groups were the same.The main points were Shiqizhui(EX-B8),Yaoyangguan(DU 3),Shenyu(BL 23),Dachangyu(BL 25),Huantiao(GB 30),Yanglingquan(GB 34).In the acupuncture+herbal medicine group,Danggui Sini Decotion and Yiguan Decotion combined with acupunctural therapy was used,while in the acupuncture group,only common acupunctural therapy was used.The treatment was performed every other day and in the acupuncture+herbal medicine group,the decoction was provided,one pack per day.A course of treatment lasted 4 weeks.The visual analog scale(VAS)scores and clinical efficacy in the two groups were compared.Results After treatment,the total effectiveness rate was 86.7%in the acupuncture+herbal medicine group,while in the acupuncture group,the rate was 71.4%.The total effectiveness rate in the acupuncture+herbal medicine group was higher than that in the acupuncture group,the difference was statistically significant(P<0.05).The VAS score after treatment in the acupuncture+herbal medicine group was lower than that in the acupuncture group with a significant difference(P<0.05).Conclusion Efficacy of Danggui Sini Decotion and Yiguan Decotion combined with acupunctural therapy for refractory low back pain is superior to that by common acupunctural therapy.
