实施集束化护理对预防肿瘤患者PICC相关性静脉血栓形成的效果

摘　　要：目的探讨集束化护理对预防肿瘤患者PICC相关性静脉血栓形成的效果。方法选取2015年3月1日～10月31日我院实施集束化护理之前PICC置管的417例患者作为对照组,选取2016年3月1日～10月31日我院实施集束化护理之后PICC置管的515例患者作为观察组。观察两组患者PICC相关性静脉血栓形成发生率和发生时间及集束化护理的依从性。结果观察组肿瘤患者PICC相关性静脉血栓形成总发生率(3.30%)低于对照组(6.24%),差异有统计学意义(P<0.05);观察组置管≥30 d的患者PICC相关性静脉血栓形成发生率低于对照组,差异有统计学意义(P<0.05);观察组集束化护理的依从性评分[(67.24±12.50)分]高于对照组[(40.15±8.67)分],差异有统计学意义(P<0.01)。结论实施集束化护理可有效降低肿瘤患者PICC相关性静脉血栓形成发生率,提高护士和患者对集束化护理的依从性,是预防肿瘤患者PICC相关性静脉血栓形成的关键,值得临床进一步推广。Objective To explore the effect of cluster nursing on prevention of PICC-related venous thrombosis in cancer patients.Methods A total of 417 patients with PICC catheter before cluster nursing in our hospital from March 1,2015 to October 31,2015 were set as the control group,and 515 patients with PICC catheter after cluster nursing in our hospital from March 1,2016 to October 31,2016 were set as the observation group.The incidence and time of PICC-related venous thrombosis and the compliance of cluster nursing care were observed in the Two groups.Results The total incidence of PICC-related venous thrombosis in tumor patients of the observation group(3.30%)was lower than that in the control group(6.24%),and the difference was statistically significant(P<0.05).In the observation group,the incidence of picc-related venous thrombosis in patients with catheterization greater than 30 d was lower than that in the control group,and the difference was statistically significant(P<0.05).The compliance score of cluster nursing in the observation group([67.24±12.50]points)was higher than that in the control group([40.15±8.67]points),and the difference was statistically significant(P<0.01).Conclusion The implementation of cluster nursing can effectively reduce the incidence of PICC-related venous thrombosis in cancer patients,improve the compliance of nurses and patients to cluster nursing,which is the key to prevent the formation of PICC-related venous thrombosis in cancer patients,and it is worthy of further clinical promotion.
