影响新生儿坏死性小肠结肠炎预后的危险因素

摘　　要：目的分析影响新生儿坏死性小肠结肠炎患儿预后情况的危险因素。方法选取2015年6月-2018年7月30例医院收治的新生儿坏死性小肠结肠炎患儿作为观察对象,采集所有患者的一般资料与临床资料,并给予对症治疗之后根据患儿的预后情况分为观察组(预后不良)与对照组(预后良好),分析影响新生儿坏死性小肠结肠炎患儿预后情况的危险因素。结果通过单因素分析法发现,两组在早产、低体重、腹膜炎、败血症、代谢性酸中毒、白细胞异常、血小板减少、C反应蛋白升高、腹部X线异常等指标方面的差异有统计学意义(P<0.05);通过进一步多因素分析法发现,败血症、腹膜炎以及代谢性酸中毒是影响新生儿坏死性小肠结肠炎患儿预后情况的高危因素(P<0.05)。结论影响新生儿坏死性小肠结肠炎患儿预后的因素有很多,尤其要警惕合并败血症、腹膜炎以及代谢性酸中毒患儿的临床观察,尽早给予对症干预措施,从而降低患儿的预后不良率。Objective to analyze the risk factors affecting the prognosis of neonatal necrotizing enterocolitis.Methods select on June 6,2015-July 2015 children with 30 cases of neonatal necrotizing enterocolitis hospitals as research object,the general data of all patients with clinical data collected,and symptomatic treatment according to the patient's prognosis after divided into observation group(prognosis)and the control group()good prognosis,analysis influence children with neonatal necrotizing enterocolitis risk factors of prognosis.Results univariate analysis showed that there were statistically significant differences between the two groups in indicators such as premature birth,low weight,peritonitis,sepsis,metabolic acidosis,leukocyte abnormality,thrombocytopenia,c-reactive protein elevation,and abdominal X-ray abnormality(P<0.05).Further multivariate analysis showed that septicemia,peritonitis and metabolic acidosis were the risk factors influencing the prognosis of neonatal necrotizing enterocolitis(P<0.05).Conclusion there are many factors affecting the prognosis of neonatal necrotizing enterocolitis,especially the clinical observation of children with sepsis,peritonitis and metabolic acidosis,and symptomatic intervention measures should be given as soon as possible,so as to reduce the prognosis rate of the children.
