摘 要:目的探讨肺小气道病变患者低剂量呼气相肺重建CT影像学特征与外周血特异性标记物沉默信息调节因子2相关酶1(Sirt-1)、Nod样受体热蛋白结构域相关蛋白3(NLRP3)、肿瘤坏死因子α(TNF-α)、白细胞介素8(IL-8)水平及转化生长因子β1(TGF-β1)表达的相关性。方法选择80例小气道病变患者为病例组,另选取80例健康体检者为对照组。对2组采用Western blotting法检测外周血单个核细胞(PBMCs)中Sirt-1、NLRP3蛋白的表达,同时运用酶联免疫吸附(ELISA)法检测2组血清TNF-α、IL-8、TGF-β1水平。探讨低剂量CT特征与外周血特异性标记物表达的相关性。结果病例组PBMCs中Sirt-1蛋白表达水平低于对照组(P <0. 05),NLRP3蛋白表达水平高于对照组(P <0. 05)。病例组血清TNF-α、IL-8、TGF-β1水平均低于对照组(P <0. 05)。将病例组根据CT影像学特征再分组比较外周血特异性标记物水平,结果显示有无空气潴留、有无磨玻璃样密度影、有无肺大泡、有无小叶中心肺气肿、有无肺实质微结节等组间的Sirt-1、NLRP3、TNF-α、IL-8、TGF-β1水平差异均有显著性(P <0. 05)。经Pearson相关分析显示,空气潴留、磨玻璃样密度影、肺大泡、小叶中心肺气肿、肺实质微结节等低剂量CT特征与外周血特异性标记物Sirt-1、NLRP3、TNF-α、IL-8、TGF-β1表达水平均呈显著正相关(P <0. 05)。结论肺小气道病变患者空气潴留等低剂量CT特征与Sirt-1等多种特异性标记物表达相关。可将低剂量CT与上述特异性标记物联用,诊断肺小气道病变。Objective To investigate the correlation between CT imaging features of lung reconstruction with low dose expiratory phase and the expression of specific markers of silent information regulator 2 related 1 enzyme(Sirt-1),Nod like receptor pyrindomain associated protein 3(NLRP3),tumor necrosis factorα(TNF-α),interleukin 8(IL-8)and transforming growth factorβ1(TGF-β1).Methods 80 cases of small airway disease were selected as the case group,and 80 healthy people were selected as the control group.The expression of Sirt-1 and NLRP3 protein in peripheral blood mononuclear cells(PBMCs)was detected by Western blotting in the two groups,and the levels of TNF-α,IL-8 and TGF-β1 were detected by enzyme-linked immunosorbent assay(ELISA).The correlation between the characteristics of low dose CT and the expression of specific markers in peripheral blood was analyzed.Results he expression of Sirt-1 protein in PBMCs was lower in the case group than in the control group(P<0.05),and the expression level of NLRP3 protein was higher in the case group than in the control group(P<0.05).The average levels of TNF-α,IL-8 and TGF-β1 in the case group were lower than those in the control group(P<0.05).The case group was subdivided into different groups according to the characteristics of CT imaging,and the results showed that there were significant differences in air retention,ground glass opacity,pulmonary bullous emphysema,centrilobular pulmonary parenchymal micronodules,Sirt-1 TNF-α,NLRP3,IL-8 and TGF-β1(P<0.05).Pearson correlation analysis showed that the characteristics of low dose CT,such as air retention,ground glass like density,pulmonary bullae,central lobular emphysema,and pulmonary parenchyma micronodules,were positively correlated with the expression of peripheral blood specific markers Sirt-1,NLRP3,TNF-a,IL-8,and TGF-β1(P<0.05).Conclusion Low dose CT characteristics such as air retention in patients with small airway lesions are associated with the expression of a variety of specific markers such as Sirt-1.The low
