摘　　要：目的讨论MSCT在胃癌术前诊断分期及手术方案选择中的应用价值。方法具有明确病理诊断结果的胃癌患者117例,对照分析患者的影像资料和病理结果,讨论影像表现与是否有转移的关系,同时观察临床诊断分期的准确性,并与患者手术方式进行对照,分析在手术方案选择中的作用。结果 CT各影像征象与术后病理是否有转移情况在淋巴结(χ~2=12.716)、癌肿门脉期强化方式(χ~2=4.447)以及浆膜形态(χ~2=5.379)的比较中,均具有统计学意义(P<0.05); CT的术前评估准确率为93.16%(109/117),手术方式与CT建议的符合度为82.86%(58/70)。结论多层螺旋CT能够在一定程度上对胃癌进行准确的术前诊断分期,并且能够显示受累脏器的附近及远端转移程度,因此能够指导临床合理治疗方案。Objective To evaluate the efficacy of MSCT in preoperative diagnosis of gastric cancer stages and selection of surgical approach.Methods Firstly,117 patients having been pathologically diagnosed as cases of gastric cancer were chosen as the subjects of study.CT images and Results from pathological diagnosis were then reviewed with a viewing to identifying any correlation of CT indications to metastasis.The accuracy of the clinical stage of cancer diagnosis was then evaluated against the surgical approach to be applied where the efficacy of MSCT in the selection of surgical approach was also evaluated.Results Statistically significant difference was found between CT imaging and pathological diagnosis in identifying post-operative metastasis in lymph nodes(χ^2=12.716),and between portal phase enhancement pattern(χ^2=4.447)and serosal morphology(χ^2=5.379)(P<0.05).The accuracy of CT preoperative evaluation was 93.16%(109/117)and the coincidence of operative Methods with CT recommendations was 82.86%(58/70).Conclusions Multi-slice spiral CT is desirably efficient in the preoperative diagnosis of stages of gastric cancer and indicating distant metastasis and metastasis in the vicinity of organs involved.It can serve as a desirable technology for preoperative diagnosis of stages of gastric cancer and selection of surgical approach.
