术前中性粒细胞与淋巴细胞比值对乳腺肿瘤的预测价值及其与Beclin1的关系

摘　　要：目的分析术前外周血中性粒细胞与淋巴细胞计数比值(NLR)在乳腺肿瘤预测中的价值,探讨NLR与自噬相关基因Beclin1在乳腺癌中的关系。方法回顾性分析2016年1月至2017年8月在该院接受手术治疗的126例原发性浸润性乳腺癌患者术前NLR,分析NLR与不同临床病理特征的关系;以及反转录-聚合酶链反应(RT-PCR)检测Beclin1 mRNA的表达,初步探讨Beclin1与NLR的相关性。结果乳腺癌组NLR明显高于乳腺良性肿瘤组,两组差异有统计学意义(P<0.05);NLR在年龄、有无淋巴结转移、Ki67表达率分组中差异有统计学意义(P<0.05),而在肿瘤直径大小、病理分级、雌激素受体、孕激素受体、人表皮生长因子受体2等其他临床病理特征中差异无统计学意义(P>0.05);RT-PCR检测Beclin1mRNA结果显示,肿瘤组织表达量明显低于癌旁组织;根据Beclin1mRNA表达差异分组分析发现,Beclin1mRNA表达下调组NLR较高,与未下调查组比较差异有统计学意义(P<0.05)。结论 NLR在乳腺肿瘤预测中有一定价值,炎性反应、自噬、肿瘤三者之间可能存在相互促进关系。Objective To analyze the value of preoperative ratio of peripheral blood neutrophil to lymphocyte count(NLR)in predicting breast cancer,and to explore the relationship between NLR and autophagy-related gene Beclin1in breast cancer.Methods The preoperative NLR values of126patients with primary invasive breast cancer treated in the hospital from January2016to August2017were retrospectively analyzed.The relationship between NLR and different clinicopathological features was analyzed.The expression of Beclin1was detected by reverse transcription-polymerase chain reaction(RT-PCR),and the correlation between Beclin1and NLR was preliminarily discussed.Results The NLR value of the breast cancer group was significantly higher than that of the benign group.There was a statistically significant difference between the two groups(P<0.05).There was a statistically significant difference in the NLR values with age,lymph node metastasis,and Ki67expression rates(P<0.05).There was no significant difference with other clinicopathological features such as tumor diameter,pathological grade,estrogen receptor,progesterone receptor and CER-2(P>0.05).RT-PCR showed that the expression of Beclin1in cancer group was significantly lower than that in adjacent group.According to the differential grouping analysis of Beclin1expression,the NLR in the down-regulated Beclin1expression group was higher than that in the adjacent group,and the difference between the two groups was statistically significant(P<0.05).Conclusion NLR has a certain value in the prediction of breast tumors,and there is a mutually promoting relationship among inflammation,autophagy,and tumor.
