成年男女泌尿系统感染临床特征、病原菌分布及药敏分析

摘　　要：目的了解成年男女泌尿系统感染特点,定期进行耐药监测,为临床合理用药和经验用药提供参考。方法使用WHONET5.6软件回顾性分析了2014年1月到2016年12月我院不同性别成年人尿路感染患者分离菌的种类、临床特征及耐药性。结果成年男性和女性患者分别分离出642株和989株,病区分布不同,成年男性和女性患者分别以泌尿外科和肾内科/风湿内分泌科为主,成年男女感染病原菌均以大肠埃希菌和肺炎克雷伯菌为主,大肠埃希菌耐药性男性显著高于女性,差异有统计学意义(P<0.05);肺炎克雷伯菌耐药性男性略高于女性,差异无统计学意义(P>0.05),妥布霉素、左氧氟沙星和呋喃妥因除外(P<0.05)。结论成年男女尿路感染以大肠埃希菌为主,实验室应定期对临床泌尿系统感染病原菌分布情况、耐药性及其感染特征进行监测和总结,为临床根据不同性别对患者合理应用抗生素提供依据。Objective To comprehend the clinical characteristics of urinary tract infection in adult male and fe?male,and regular monitoring of drug resistance to provide reference for clinical rational and experiential drug use.Method Retrospective analysis the strains isolated from patients with urinary tract infection in adult male and fe?male from2014to2016.Result642and989strains were isolated from adult male and female,the mainly depart?ment distribution were Urinary surgery and Nephrology/Rheumatic department of endocrinology,the main pathogens were Escherichia coli and Klebsiella pneumoniae.The drug resistance of Escherichia coli in Adult male were significantly higher than adult female(P<0.05),and the drug resistance of Klebsiella pneumoniae in adult male were slightly higher than adult female(P>0.05),except Tobramycin,Levofloxacin and Furazolidin(P<0.05).Conclusion Urinary tract infections in adult male and female are mainly Escherichia coli.The laboratory should regularly analyzed and summarized the distribution of pathogens in clinical urinary tract and drug resis?tance and infection characteristics,to guide the clinical rational drug use according to different sex patients.
