817例住院患者糖皮质激素类药物临床应用情况与合理性分析

摘　　要：目的:了解郑州人民医院(以下简称"我院")糖皮质激素类药物临床应用情况。方法:统计2017年第4季度我院销售金额排序居前10位的糖皮质激素类药物,对使用上述10种药品的出院患者病历817份进行汇总分析;根据《中华人民共和国药典·临床用药须知》、《糖皮质激素类药物临床应用指导原则》(卫办医政发[2011]23号)及我院《糖皮质激素类药物临床使用管理办法》进行用药合理性评价。结果:2017年第4季度我院销售金额排序居前5位的糖皮质激素类药物为氢化泼尼松注射液(2 ml∶10 mg)、注射用甲泼尼龙琥珀酸钠(甲强龙,40 mg)、布地奈德鼻喷雾剂[64μg/喷(1.28 mg/ml)]、注射用甲泼尼龙琥珀酸钠(甲强龙,0.5 g)、注射用甲泼尼龙琥珀酸钠(尤米乐,40 mg);糖皮质激素类药物的主要给药途径为静脉滴注、雾化吸入和口服,主要用于呼吸系统疾病、自身免疫性疾病、肾脏疾病、严重感染或炎症反应等疾病的治疗;糖皮质激素类药物不合理应用类型主要为用法与用量不适宜、联合用药不适宜和药品选择不适宜等。结论:针对糖皮质激素类药物不合理应用情况,应加强监测评估力度,促进临床合理用药。OBJECTIVE:To investigate the clinical application of glucocorticoid drugs in Zhengzhou People’s Hospital(hereinafter referred to as“our hospital”).METHODS:The top10glucocorticoid drugs ranked by consumption sum in our hospital in the fourth quarter of2017were collected,and analysis was conducted on the data of817cases of medical records.According to“Chinese Pharmacopoeia,Clinical Medication Instructions”,“Guidelines for Clinical Application of Glucocorticoids”(Health,Medical and Political Bureau〔2011〕No.23)and“Management of Clinical Application of Glucocorticoids”in our hospital,rationality evaluation was conducted.RESULTS:The top5glucocorticoid drugs ranked by consumption sum in the fourth quarter of2017were respectively prednisolone injection(2ml∶10mg),methylprednisolone sodium succinate injection(Solu-medrol,40mg),budesonide nasal spray[64μg/spray(1.28mg/ml)],methylprednisolone sodium succinate injection(Solu-medrol,0.5g),methylprednisolone sodium succinate injection(YouMiLe,40mg).The main routes of administration of glucocorticoids were intravenous drip,aerosol inhalation and oral administration,mainly for the treatment of diseases such as respiratory diseases,autoimmune diseases,kidney diseases,serious infections or inflammatory reactions.The irrational application types of glucocorticoid drugs mainly performed as inappropriate usage and dosage,unsuitable drug combination and improper drug selection.CONCLUSIONS:In view of the irrational application of glucocorticoid drugs,monitoring and evaluation should be strengthened to promote the clinical rational drug application.
