球囊拘禁与导丝保护在真性冠脉分叉病变介入治疗中的短期疗效观察

摘　　要：目的评价球囊拘禁技术(JBT)与传统的导丝保护技术(JWT)在冠脉分叉病变介入治疗中的安全性和有效性。方法 256例经冠脉造影证实为冠脉真性分叉病变的患者,在PCI前随机分为球囊拘禁组和导丝保护组,所有患者先采用主支植入支架的术式,分支用球囊拘禁保护者为试验组,分支用导丝保护者为对照组,观察两组在支架植入术后因斑块和界嵴移位,分支夹层等导致分支受累加重或闭塞的发生率,以及因分支受累改变成必要时T术式(TAP)的发生率。结果与对照组相比,试验组术后TIMI3级血流明显优于对照组,分支受累和闭塞的发生率明显降低,同时因分支受累改变成必要时T术式(TAP)的发生率明显低于对照组,两组差异有统计学意义(P<0.05)。结论冠脉真性分叉病变介入治疗时采用球囊拘禁技术较传统的导丝保护对分支保护具有较好的安全性和有效性。Objective To evaluate the safety and effectiveness of jailed balloon technique(JBT)and conventional jailed wire technique(JWT)in the intervention of coronary bifurcation lesions.Methods 256 cases of patients with true coronary bifurcation lesions confirmed by coronary angiogram were randomly divided into JBT group and JWT group before percutaneous coronary intervention(PCI).All patients were firstly implanted with stents at coronary trunk and branch was protected by JBT in experimental group,while in control group,branch was protected by JWT.The incidence of aggravated involvement and occlusion of branch resulted from plaque as well as carina shift,branch dissection,et cetera after PCI,and the incidence of operation changed to TAP due to branch involvement,were compared between the two groups.Results After operation,TIMI grade3blood flow of experimental group was prior to control group,incidence of branch involvement and occlusion in experimental group decreased significantly and meanwhile,incidence of operation changed to TAP due to branch involvement in experimental group was significantly lower than that in control group,the differences being statistically significant(P<0.05).Conclusion JBT has better safety profile and effectiveness in terms of branch protection than conventional JWT in the intervention of true coronary bifurcation lesions.
