摘 要:目的对比在中等浓度七氟烷诱导下,不使用肌松剂时1. 5mg/kg丙泊酚与使用0. 6mg/kg罗库溴铵2~8岁儿童的气管插管条件。方法选择50名拟行短小择期手术,年龄2~8岁的患儿,ASA1~2级,随机分配至P组(丙泊酚组)或R组(罗库溴铵组)。术前30分钟口服咪唑安定0. 5mg/kg,用8%七氟醚诱导麻醉,建立静脉通道后,静脉注射芬太尼1μg/kg,同时将七氟醚浓度降至4%,根据分组,分别静脉给与等容量生理盐水+1. 5mg/kg丙泊酚或0. 6mg/kg罗库溴铵+等容量的生理盐水。四分钟后行气管插管,记录插管条件(可接受/良好),插管时间和血流动力学参数。结果两组间可接受/极好的插管条件发生率之间无统计学差异(P=1)。两组间插管时间无统计学差异(P组26. 84±5. 67s,R组25. 76±4. 98s,P=0. 711),两组间呼吸末七氟烷浓度间无统计学差异(P组3. 3±0. 30%,R组3. 1±0. 2%,P=0. 11)。结论在中等浓度诱导的小儿手术中,丙泊酚(1. 5mg/kg)与罗库溴铵(0. 6mg/kg)所提供的插管条件无差异。Objective To compare the tracheal intubation conditions of children aged2~8years with propofol(1.5mg/kg)and rocuronium(0.6mg/kg)without muscle relaxant induced by moderate concentration of sevoflurane.Methods A total of50children aged2to8years,scheduled for short term elective surgery,grade ASA1-2,were selected and randomly assigned to P group(propofol group)or R group(rocuronium group).Midazolam0.5mg/kg was administered orally30minutes before surgery and8%sevoflurane was used to induce anesthesia.After the establishment of intravenous channel,fentanyl was injected intravenously at a dose of1μg/kg and the concentration of sevoflurane was reduced to4%.According to the grouping,intravenous administration of equal amount of normal saline+1.5mg/kg propofol or0.6mg/kg rocuronium+equal amount of normal saline was given respectively.Tracheal intubation was performed4minutes later.Intubation conditions(acceptable/good),intubation time and hemodynamic parameters were recorded.Results There was no significant difference in the incidence of acceptable/excellent intubation conditions between the two groups(P=1).No significant difference in intubation time between the two groups(group P26.84±5.67s,group R25.76±4.98s,P=0.711),and no significant difference in the concentration of sevoflurane at the end of respiration between the two groups(group P3.3±0.30%vs group R3.1±0.2%,P=0.11).Conclusion There was no difference in intubation conditions between propofol(1.5mg/kg)and rocuronium(0.6mg/kg)in moderate concentration induced pediatric surgery.
