摘  要:目的探讨川崎病(KD)患儿N端脑钠肽前体(NT-proBNP)测定与冠状动脉损害(CAL)的关系。方法选取2015年3月至2017年1月在四川大学华西第二医院住院的川崎病儿童,对其进行冠状动脉超声检查,根据检查结果分为冠状损害组((CAL,n=52)和未合并其他疾病且无冠状动脉损害组(NCAL,n=119),选取发热5天以上的支气管肺炎为对照组(n=52)。采集患儿静脉血标本,采取免疫化学发光法分别检测其NT-proBNP水平。采用非参数检验Mann-Whitney U检验和x^2检验比较各组间NT-proBNP水平是否有显著差异。结果对照组、NCAL组及CAL组NT-proBNP(pg/ml)水平四分位间距分别为151. 00(63. 70,383. 75)、543. 00(226. 00,1660. 00)、1013. 00(259. 00,2882. 50); NCAL组及CAL组均明显高于对照组,差异有统计学意义(P<0. 05); CAL组NBNP明显高于NCAL组,差异有统计学意义(P<0. 05)。结论川崎病患儿血浆NT-proBNP水平明显升高,并且与患儿冠状动脉损害有密切关系,检测NT-proBNP可以为川崎病及其早期是否发生冠脉损害的诊断和预防提供一定的帮助。Objective To explore the relationship between the detection of N-terminal brain natriuretic peptide precursor( NT-proBNP) and coronary artery lesion( CAL) in children with kawasaki disease( KD). Methods Children with kawasaki disease hospitalized in West China Second Hospital of Sichuan University from March 2015 to January 2017 were selected and done with coronary artery ultrasound examination who were divided into coronary artery lesion group(( CAL),n = 52) and none coronary artery lesion group with no other disease( NCAL,n = 119) according to the inspection results,selected fever more than 5 days for the control group( n = 52) with bronchial pneumonia.Venous blood samples were collected from the children and their NT-proBNP levels were detected by immunochemiluminescence. Non-parametric tests Mann-Whitney U test and the χ^2 test were used to compare whether there were significant differences in NT-proBNP levels between groups. Results The interquartile range of NT-proBNP( pg/mL) levels in the control group,NCAL group and CAL group were 151. 00( 63. 70,383. 75),543. 00( 226. 00,1660. 00) and1013. 00( 259. 00,2882. 50) respectively.Both the NCAL group and the CAL group were significantly higher than the control group,and the difference was statistically significant( P < 0. 05). The NBNP of the CAL group was significantly higher than that of the NCAL group,and the difference was statistically significant( P<0. 05).Conclusion The plasma NT-proBNP level of children with kawasaki disease is significantly increased,which is closely related to the coronary artery lesion. Detection of NT-proBNP can provide certain help for the diagnosis and prevention of kawasaki disease and its early occurrence of coronary artery lesion.
