摘 要:目的探讨呼出气一氧化氮(FeNO)在儿童哮喘控制中的评估作用,及其与肺功能指标、外周血嗜酸性粒细胞(EOS)和免疫球蛋白E(IgE)的相关性。方法 2015年12月至2016年12月于该院儿童哮喘专科门诊就诊及定期复诊的5~14岁哮喘患儿,根据哮喘控制水平不同分为完全控制组、部分控制组及未控制组,以健康儿童作为对照组,各20例。检测并比较FeNO、第一秒用力呼气容量(FEV1)、呼气流速峰值(PEF)、外周血EOS和IgE水平,分析Fe-NO与其他指标的相关性。结果未控制组、部分控制组及完全控制组FeNO水平高于对照组(P<0.05)。对照组、完全控制组、部分控制组间FEV1水平比较差异无统计学意义(P>0.05),各组与未控制组比较差异有统计学意义(P<0.05)。对照组与完全控制组间PEF比较差异无统计学意义(P>0.05),但对照组、完全控制组与部分控制组、未控制组比较差异均有统计学意义(P<0.05)。部分控制组FeNO与FEV1、PEF呈正相关(P<0.05),但与IgE、EOS无明显相关性(P>0.05);其余各组FeNO与FEV1、PEF、IgE、EOS均无相关性(P>0.05)。结论 FeNO与哮喘控制水平有关,与肺功能指标、IgE、EOS联合应用可更好地评估哮喘控制水平。Objective To evaluate the value of fractional exhaled nitric oxide(FeNO)in the control of childhood asthma and the relationship between FeNO and forced expiratory volume in one second(FEV1),peak expiratory flow rate(PEF),immunoglobulin E(IgE)and eosinophilic granulocytes(EOS).Methods Children aged 5-14 years old with asthma were recruited and classified into three groups,including fully controlled group,partially controlled group and uncontrolled group.Healthy children were enrolled as control group,20 cases ineach group.Levels of FeNO,lung function,EOS and IgE were detected,and the correlation between these indicators were analyzed.Results The initial levels of FeNO in uncontrolled group,partially controlled group and fully controlled group were higher than control group(P<0.05).The initial levels of FEV1 in control group,fully controlled group and partially controlled group were without significant differences(P>0.05),but were higher than uncontrolled group(P<0.05).There was no significant difference of PEF between control group and fully controlled group(P>0.05),but those of the two groups were higher than partially controlled group and uncontrolled group(P<0.05).FeNO was only positively correlated with FEV1 and PEF in partially controlled group(P<0.05).Conclusion FeNO could be closely correlated with asthma control levels and should be combined applied with FEV1,PEF,IgE and EOS for comprehensive assessment of asthma.
