摘  要:目的比较I型呼吸衰竭新生儿在使用两种经鼻正压通气方法时导致鼻部压疮的观察。方法将60例经过血气分析确诊I型呼吸衰竭的新生儿随机分成对照组32和观察组28例。对照组使用经鼻持续气道正压通气(Nasal Continous Positive Airway Presure,NCPAP);观察组使用加温湿化高流量鼻导管通气(heated humidified high flow nasal cannula,HHHFNC)。观察两组患儿鼻部压疮的发生情况。结果观察组鼻部压疮发生率明显小于对照组。差异有统计学意义(P<0.01)。结论加温湿化高流量鼻导管通气治疗I型新生儿呼吸衰竭时可明显减少患儿并发鼻部压疮。Objective To compare the observation of neonates with type Ⅰ respiratory failure in the use of two nasal pressure positive ventilation methods.Methods Sixty newborns diagnosed with type I respiratory failure through blood gas analysis were randomly divided into control group(32)and observation group(n=28).Nasal Continous Positive Airway Presure(NCPAP)was used in the control group.The observation group was treated with humidified high flow nasal cannula(HHHFNC).The incidence of nasal pressure ulcer was observed in both groups.Results The incidence of nasal pressure ulcer was significantly lower in the observation group than in the control group.The difference was statistically significant(P<0.01).Conclusion The treatment of type I neonatal respiratory failure by heating and humidating high-flow nasal catheter ventilation can significantly reduce the incidence of nasal pressure.
