76例小龙虾相关横纹肌溶解综合征临床分析

摘　　要：目的探讨小龙虾相关横纹肌溶解综合征患者的临床特征、流行病学特点、治疗和预后。方法收集因食用小龙虾后发生横纹肌溶解综合征,于武汉市同济医院就诊的患者76例。观察分析其临床症状、实验室检测指标、流行病学特点、治疗和预后。结果小龙虾相关横纹肌溶解综合征临床症状主要为全身或局部肌肉酸痛、无力、尿色改变、胸闷/痛等。实验室检测显示出现血肌酸激酶、肌红蛋白、谷草转氨酶水平异常升高的患者比例均超过90%。横纹肌溶解综合征相关指标肌酸激酶水平与乳酸脱氢酶、肌红蛋白、谷丙转氨酶、谷草转氨酶水平之间呈正相关。小龙虾相关横纹肌溶解综合征患者发病集中在一年中的7月和8月,其他月份较少发生。结论小龙虾相关横纹肌溶解综合征以肌肉酸痛为主要临床症状,严重并发症少,通过补液、碱化尿液等治疗预后良好。Objective To investigate the clinical features,epidemiological characteristics,treatment and prognoses of crayfish-related rhabdomyolysis syndrome.Methods Totally,76cases with rhabdomyolysis syndrome resulting from eating crayfish were collected in Tongji Hospital.Their clinical symptoms,laboratory test results,epidmiological features,treatment and prognosis were analyzed.Results The main clinical symptoms of the patients with crayfish-related rhabdomyolysis syndrome were systemic or local muscle soreness,weakness,urine color change,chest distress/pain,etc.Laboratory tests indicated that over90%cases had abnormal creatine kinase,myoglobin,and aspartate aminotransferase level,respectively.Serum creatine kinase level was positively associated with lactate dehydrogenase,myoglobin,alanine aminotransferase,and aspartate aminotransferase level,respectively.The presence of crayfish-related rhabdomyolysis syndrome was mainly in July and August of the year,and all patients had a good prognosis.Conclusion The main clinical symptom of all patients with crayfish-related rhabdomyolysis syndrome is myalgia.All patients have good prognosis with less severe complications through therapeutic regimens including supplementing fluid,alkalizing urine.
