摘　　要：目的研究噻托嗅铵粉吸入剂(思力华)联合呼吸训练对稳定期慢性阻塞性肺疾病(COPD)的疗效。方法 126例稳定期COPD患者平均分2组,对照组给予每日1次、每次1吸的噻托溴铵粉吸入剂治疗,观察组在此基础上联合呼吸训练。检测患者的动脉血气及肺功能指标、血清白细胞介素(IL)-8及肿瘤坏死因子(TNF)-α水平,评估COPD评估测试问卷(CAT)、圣乔治呼吸问卷(SGRQ)、焦虑自评量表(SAS)及抑郁自评量表(SDS)评分。结果相较于对照组,治疗2年后,观察组患者的动脉血氧分压(PaO_2)、动脉血氧饱和度(SaO_2)更高(P <0.05),动脉血CO_2分压(PaCO_2)更低(P <0.05);第1秒用力呼气量(FEV1)、FEV1占预计值百分比(FEV1)(%)、FEV1/用力肺活量(FVC)更高(P <0.01);血清IL-8和TNF-α水平、CAT、SGRQ、SAS及SDS评分更低(P <0.01)。结论噻托溴铵粉吸入剂联合呼吸训练对稳定期COPD患者具有更好的治疗效果,其疗效优于噻托溴铵粉吸入剂单一用药。Objective To research the efficacy of tiotropium bromide powder for inhalation treatment combined with breathing training on stable chronic obstructive pulmonary disease(COPD).Methods A total of126patients with stable COPD were divided into two groups.For the control group,patients were treated with tiotropium bromide powder for inhalation,and patients of observation group were subjected to tiotropium bromide powder for inhalation treatment combined with breathing training.The arterial blood gas index,pulmonary function index,and serum IL-8and TNF-αlevels were detected.CAT,SGRQ,SAS and SDS scores were evaluated.Results After2years treatment,compared with control group,patients of observation group were with obviously higher PaO2and SaO2(P<0.05),much lower PaCO2(P<0.05),remarkable higher FEV1,FEV1(%)and FEV1/FVC(P<0.01),significantly lower serum IL-8and TNF-αlevels(P<0.01),as well as drastically lower CAT,SGRQ,SAS and SDS scores.Conclusion tiotropium bromide powder for inhalation treatment combined with breathing training has a better therapeutic effect on patients with stable COPD than single medication of tiotropium bromide powder for inhalation.
