浆细胞性乳腺炎185例临床分析

摘　　要：目的探讨浆细胞性乳腺炎的临床诊断与治疗方法。方法对成都市第五人民医院2011年1月至2017年2月收治的浆细胞性乳腺炎185例患者的临床资料进行回顾性分析。结果 185例患者术后病理诊断均为浆细胞性乳腺炎,其中2例伴中度不典型增生。185例患者中乳房局部伴有脓肿形成者22例,行脓肿切开引流术,待完全愈合后1个月再行病灶切除和乳头成形术;局部红肿未形成脓肿者163例,其中70例采用红外线+金黄如意散+红金消结胶囊治疗,40例采用红外线+金黄如意散治疗,53例采用红外线治疗,待血象恢复正常,红肿消失后立即行病灶切除和乳头成形术治疗。185例患者术后随访3个月至6年均未见复发。结论浆细胞性乳腺炎炎症期予以中西医结合治疗,尤其金黄如意散外用,红金消结胶囊口服的应用能明显提高疗效;炎症缓解后予以手术治疗是浆细胞性乳腺炎根治的有效措施,应在局部炎症控制后尽早手术治疗,手术必须切除病灶,翻转乳头乳晕,术后定期随访。Objective To investigate the clinical diagnosis and treatment of plasma cell mastitis(PCM).Methods Clinical data of185PCM patients who were admitted to Chengdu Fifth People’s Hospital between January2011and February2017were analyzed retrospectively.Results All the185patients were diagnosed as having PCM by postoperative pathology,including2patients with moderate atypical hyperplasia.Twenty-two patients with local abscess formation underwent incisional drainage,and then received lesion resection and mammaplasty1month after complete healing.Among the163non-abscess patients with local swelling,70were treated with infrared radiation,Jinhuangruyi pulvis and Hongjinxiaojie capsule,40were given infrared radiation and Jinhuangruyi pulvis,and53only received infrared radiation.Lesion resection and mammaplasty were performed immediately after the hemogram returned to normal and the swelling disappeared.No recurrence occurred in all patients during follow-up of3months to6years.Conclusion The combination of traditional Chinese medicine and Western medicine,especially the use of Jinhuangruyi pulvis and Hongjinxiaojie capsule during inflammatory stage,can significantly improve the outcome in patients with PCM.After inflammation resolution,as an effective method for radical treatment of PCM,surgery should be carried out as soon as possible to remove the lesion and reverse the nipple and areola after local inflammation is controlled.Furthermore,postoperative follow-up should be performed regularly.
