子宫内膜异位症患者手术前、后血浆VEGF、H_2S水平的临床研究

摘　　要：目的:探讨子宫内膜异位症(EMs)患者手术前、后血浆血管内皮生长因子(VEGF)、硫化氢(H_2S)水平的变化及其与EMs临床分期和复发的关系。方法:以经手术确诊的120例EMs患者为研究对象,所有患者在术前、术后1个月、术后3个月、术后6个月及术后1年时留取血样。采用酶联免疫吸附法检测血浆VEGF水平,采用分光光度法检测血浆H_2S水平。结果:患者手术后VEGF、H_2S水平均低于手术前,差异有统计学意义(P均<0.05);患者手术后3个月VEGF、H_2S水平均低于手术后1个月,差异有统计学意义(P均<0.05)。EMsⅢ、Ⅳ期患者手术前VEGF、H_2S水平均高于EMsⅡ期患者,差异有统计学意义(P均<0.05);EMsⅣ期患者手术前VEGF、H_2S水平均高于EMsⅢ期患者,差异有统计学意义(P均<0.05);术后1年,复发患者VEGF、H_2S水平均高于未复发患者,差异有统计学意义(P均<0.05)。结论:EMs患者手术后血浆VEGF、H_2S水平显著下降,重度EMs患者较轻度EMs患者VEGF、H_2S水平高,复发患者VEGF、H_2S水平明显升高。VEGF、H_2S可用于评价EMs的治疗效果,可作为监测复发的指标。Objective:To investigate the changes of plasma vascular endothelial growth factor(VEGF)and hydrogen sulfide(H 2S)levels in patients with endometriosis(EMs)before and after operation and their relationship with the clinical staging and recurrence.Methods:120 patients with EMs were included.Blood samples were taken from all patients before,1 month,3 months,6 months and 1 year after operation.Plasma VEGF and H 2S levels were detected by enzyme linked immunosorbent assay and spectrophotometry method respectively.Results:The level of VEGF,H 2S after operation were significantly lower than that before operation(P<0.05),and the level of VEGF,H 2S in 3 months after operation were lower than that in 1 month after operation(P<0.05).The level of VEGF,H 2S before operation in patients with EMs stagingⅢ,Ⅳwere higher than that in patients with EMs stagingⅡ(P<0.05).The level of VEGF,H 2S in patients with EMs stagingⅣbefore operation were higher than that in patients with EMs stagingⅢ(P<0.05),and the level of VEGF,H 2S in recurrent patients were higher than that in patients without recurrence after operation(P<0.05).Conclusion:The plasma VEGF,H 2S level decreased significantly in EMs patients,and the VEGF,H 2S level in severe EMs patients were higher than that in mild EMs patients.The VEGF,H 2S level in recurrent patients were significantly higher.VEGF and H 2S could be used to evaluate the therapeutic effect of EMs,and could be used as an index to monitor the recurrence.
