健脾益肾养阴方辅助血液透析法对终末期糖尿病肾病细胞免疫功能的影响

摘　　要：目的探讨健脾益肾养阴方辅助血液透析法对终末期糖尿病肾病患者细胞免疫功能的影响。方法选择糖尿病肾病患者90例,分为观察组(n=45)和HD组(n=45)。另选择体检健康志愿者30例为对照组。HD组予以血液透析法治疗,观察组在进行血液透析的同时给健脾益肾养阴方辅助治疗,观察组和HD组每次血液透析参数相同。观察比较两组血液透析前、后外周血单个核细胞中CD_3^+、CD_4^+、CD_8^+、CD_4^+/CD_8^+百分比的变化。结果观察组及HD组患者透析前CD_3^+、CD_4^+、CD_4^+/CD_8^+比值均明显低于对照组(P<0.01),其中观察组及HD组患者透析前CD_8^+明显高于对照组(P<0.05);观察组及HD组两组患者透析前比较CD_3^+,CD_4^+,CD_8^+,CD_4^+/CD_8^+差异无统计学意义(P>0.05)。观察组与HD组患者透析后CD_3^+、CD_4^+、CD_4^+/CD_8^+较透析前升高,差异有统计学意义(P<0.05),其中观察组CD_3^+、CD_4^+、CD_4^+/CD_8^+较HD组升高更为明显(P<0.01),CD8^+差异无统计学意义(P>0.05)。结论健脾益肾养阴方辅助血液透析法可有效改善终末期糖尿病肾病血液透析患者的细胞免疫功能。Ojbective To investigate the effect ofJianpi Yishen Yangyin Formula assisting with hemodialysison the cellular immune function of patients with end-stage diabetic nephropathy.Methods Ninety patients with diabetic nephropathy were selected and divided into observation group(n=45)and HD group(n=45).Another30healthy volunteers were selected as the control group.The HD group was treated with hemodialysis.The observation group was treated with spleen and kidney and nourishing Yin while performing hemodialysis.The hemodialysis parameters of the observation group and the HD group were the same.The changes of CD3^+,CD4^+,CD8^+,CD4^+/CD8^+percentages in peripheral blood mononuclear cells before and after hemodialysis were compared between the two groups.Results The ratios of CD3^+,CD4^+and CD4^+/CD8^+in the observation group and HD group were significantly lower than those in the control group(P<0.01).The CD8+in the observation group and HD group was significantly higher than that in the control group(P<0.05).There were no significant differences in CD3^+,CD4^+,CD8^+,CD4^+/CD8^+between the two groups in the group and the HD group(P>0.05).The CD3^+,CD4^+,and CD4^+/CD8^+levels in the observation group and the HD group were significantly higher than those before dialysis(P<0.05).The CD3^+,CD4^+,CD4^+/CD8^+in the observation group were significantly higher than those in the HD group.(P<0.01),there was no significant difference in CD8^+(P>0.05).Conclusion Jianpi Yishen Yangyin Formula assisting with hemodialysiscan effectively improves the cellular immune function of patients with end-stage diabeticnephropathy undergoing hemodialysis.
