探究欣普贝生与COOK双球囊促宫颈成熟的效果及安全性

摘　　要：目的探究欣普贝生与双球囊对促进宫颈成熟的效果及安全性。方法方便搜集该院妇产科2017年1月—2018年4月诊治的115例宫颈Bishop评分<6分,孕周37～42周,有医学指征必须终止妊娠的孕妇,有阴道试产指征,排除引产禁忌证,随机分为两组,观察组给予欣普贝生置阴道后穹窿促宫颈成熟,参照组给予COOK双球囊促宫颈成熟,观察两组病人促宫颈成熟情况、产程进展情况、分娩情况、妊娠结局及并发症发生情况。结果不同引产方式干预后,对比两组促宫颈成熟的有效率,观察组96.55%(56/58)与参照组82.46%(47/57)相比,差异有统计学意义(χ~2=6.11,P<0.05);用药前,两组孕妇宫颈成熟程度Bishop评分(3.89±0.27)分、(3.81±0.24)分相比,差异无统计学意义(t=0.49,P>0.05)。用药12 h后,两组孕妇的宫颈成熟程度Bishop评分显著高于用药前,且观察组孕妇的宫颈成熟程度Bishop评分(8.74±1.39)分显著高于参照组(6.94±1.14)分,差异有统计学意义(t=4.52,P<0.05);对比两组孕妇宫缩过强、胎膜早破、急产、胎儿窘迫、新生儿窒息等安全性问题发生率,观察组18.97%(11/58)与参照组7.02%(4/57)相比,差异有统计学意义(χ~2=3.62,P<0.05)。结论欣普贝生的促宫颈成熟效果优于双球囊,而且总产程短于双球囊,但容易出现宫缩过强、胎膜早破、急产、胎儿窘迫等安全性问题,必须严密观察,二者各有利蔽,建议酌情选用。Objective To study the effect and safety of dinoprostone and COOK double balloon in promoting the cervical ripening.Methods115cases of delivery women from January2017to April2018whose cervix Bishop score<6marks,and gestational week was37-42weeks,and who must terminate the pregnancy due to the medical signs,were convenient randomly divided into two groups,the observation group used the dinoprostone for promoting the cervical ripening,while the control group used the COOK double balloon in promoting the cervical ripening,and the cervical ripening promotion condition,labor progression condition,delivery condition,gestational outcome and occurrence of complications of the two groups were observed.Results After the intervention of different labor ways,the effective rate of promoting the cervical ripening in the observation group and in the control group was respectively96.55%(56/58) and 82.46%(47/57),and the difference was obvious statistically significant(χ^2=6.11,P<0.05);before medication,the cervical ripening degree Bishop score of the two groups was obviously higher than that before medication[(3.89±0.27)points vs(3.81±0.24)points],the there was no statistically significant inthe difference(t=0.49,P>0.05),and after,12h of medication the cervical ripening degree Bishop score in the observation group was obviously higher than that in the control group,[(8.74±1.39)points vs(6.94±1.14)points],and the difference was statistically significant(t=4.52,P<0.05);and the incidence rate of safety problems in the observation group and in the control group was respectively18.97%(11/58)and7.02%(4/57),and the difference was statistically significant(χ^2=3.62,P<0.05).Conclusion The effect of dinoprostone in promoting the cervical ripening is better than that of COOK Double balloon,and the total labor process is shorter than that of double balloon,but the safety problems such as excessive uterine contraction,premature rupture of membranes,acute labor,fetal distress are easy to occur,and we must closely observe these problems,a
